General advice for questions on pattern diagnosis -Pick the best and most seemingly obvious answer based on the choices given. It doesn’t have to be stated the exact way you might have phrased it. In other words, maybe you are thinking the answer should be spleen yang deficiency, but that is not an...

General advice for questions on pattern diagnosis -Pick the best and most seemingly obvious answer based on the choices given. It doesn’t have to be stated the exact way you might have phrased it. In other words, maybe you are thinking the answer should be spleen yang deficiency, but that is not an option. However, there is an option that says middle jiao deficiency. Assuming the other answers are completely different, middle jiao deficiency is a close enough answer to what you were thinking. After all, yang deficiency is deficiency, and the spleen is a middle jiao organ. -Look at the complete answer. Sometimes part of an answer is correct, but another part is not. For example, perhaps a person has damp-heat in the liver and gallbladder channels. Let’s say option A says damp-heat in the liver and gallbladder channels with kidney yin deficiency and option B says Damp-heat in liver channel, go with option B. Don’t always pick the more inclusive answer. If there aren’t signs of kidney yin xu such as low back pain, nightsweats, red/peeled tongue, etc, then that should not be part of your answer. -Above all else, focus on tongue and pulse. Your pattern choice needs to be consistent with tongue/pulse. i.e. red tongue with peeled, peeling, scanty, or no coating; thin, rapid pulse is some type of yin deficiency. -Remember pulse positions: Left cun=Heart/Small Intestine Right cun=Lung/Large Intestine Left guan=Liver/Gallbladder Right guan=Spleen/Stomach Left Chi=Kidney/UB Right chi=Kidney/San Jiao Example: Deep and tight in right guan position likely means some type of middle jiao cold -Areas of tongue: Tip=Heart Area just proximal to tip=Lung Sides=Liver and GB Center=Spleen and Stomach Back=Kidney If the question specifies a particular area of the tongue having a certain characteristic, that implies the organ(s) within that area is/are part of the pattern. Example: Red sides of the tongue imply liver and/or GB heat. 8 principles 1. Heat vs cold 2. interior vs exterior 3. excess vs deficient 4. GB: Indecisive, timid, lack of courage, easily startled Blood Deficiency Pale, dry, thin Thin, fine, thready, Liver: Pale/dry skin, dizziness, blurred vision, hollow(implies dry eyes, floaters, scanty menses, dry, pale, extreme blood brittle nails, stiff joints loss) Heart: pale skin, insomnia, anxiety, If qi AND blood palpitations, poor memory def, may feel soggy pulse (often can be felt when dampness is also involved) Yang Deficiency Pale, white Weak/forceless, Kidney: Low back pain/knee pain, cold coat, deep, slow extremities, copious, clear, freq urination, wet/glossy, edema esp in legs, diarrhea, low libido, puffy Hidden, minute infertility, impotence, spermatorrhea Spleen: Prolapse, watery diarrhea with undigested food(Cock’s crow-Can be spleen/kid yang def where kid yang fails to warm spleen), abdominal pain better with warmth and pressure Heart: Chest bi or chest pain Yang collapse: Think cardiovascular shock(cold limbs, cold, oily sweat, bluish discoloration) Yin Deficiency Red, peeled Thin/rapid, Kidney: (night sweats, 5 center heat, malar coating (or little (sometimes flush, afternoon fevers), low back and knee (heat signs can be coating), dry, described as pain, scanty, dark yellow urine mild or severe) cracks floating—think not enough yin to anchor yang) Lung: dry cough, hoarse voice, sore throat, dry nose, mouth, throat Stomach: Low appetite, fullness after eating small amounts, dry mouth, scanty vomit Yin collapse: Think heat stroke (hot skin, hot sticky sweat, confusion) Essence Deficiency Causes: poor Preheaven: (Kidney)…. nutrition, overtaxation, stress, with males Postheaven: (Lung, Spleen, Stomach) excess ejaculation, with females, too many child births, S/S: Failure to thrive, impaired growth, not breast feeding, hitting milestones, fontanels don’t close, leakages of any premature aging, essence fails to engender type(i.e. marrow leading to problems with intellect, spermatorrhea) dizziness, poor memory Qi Stagnation Wiry(wiry can Liv: Hypochondriac pain or fullness, indicate qi stag, irritability, impatience, sighing, dull achy pain, Liv/GB prob) wandering pain, fatigue better with exercise. MJ: abdominal distension, bloating, gas, belching, improved by belching/passing gas Blood Stagnation Purple with choppy Sharp, stabbing, fixed pain, worse with distended pressure, masses, menstrual pain better sublingual veins with flow Food Stagnation Abdominal or epigastric distension or fullness, lack of desire to eat, regurgitation, sour taste, foul breath, belching Excess Heat Red, thick, dry, Deep, rapid, Heart Heat/Fire: Insomnia, palp, delirium, yellow coating, forceful, surging, mouth and tongue ulcers, red tip of tongue thorns flooding -Sometimes signs like delirium and mania are labelled as pericardium heat rather than heart heat/fire. Liver Heat/Fire: HA, anger, red eyes, dizziness, irritability Liver Yang Rising: Throbbing or pulsating HA, dizziness, Flushing of face Stomach Heat/Fire: Ravenous appetite, foul breath, thirst, dry mouth, can be vomiting after eating, ulcerations in mouth LI Heat: Constipation with dry, hard stools Excess Cold Pale, thick Deep, Tight, (slow) N/V, pain worse with pressure, diarrhea white coating Damp- Red, sticky, Rapid, Lung: Cough, with yellow, sticky, diff to Heat/Phlegm Heat slimy, greasy, slippery(remember expectorate sputum yellow coat that slippery pulse can indicate Middle Jiao: bloating, distension, pregnancy also) diarrhea(pus, burning, foul smelling), nausea, vomiting Heart: delirium, cloudy thinking, mania UB: UTI(Difficult, painful, burning, cloudy, dark yellow urination) Damp-Cold/Phlegm Pale, slimy, slippery Lung: Cough with clear, thin, white, copious, Cold greasy, sticky easily expectorated sputum, person may be white/grayish cold with cold limbs coating -Damp-phlegm: Cough with copious white phlegm, easily expectorated sputum, nausea, feeling of heaviness Middle Jiao: Nausea/Vomiting, bloating, diarrhea with mucus Heart: confusion, cloudy thinking, depression. Damp---Middle Jiao/Lower Jiao, can be externally contracted or internally generated Phlegm—Upper Jiao, phlegm tends to be thicker, more turbid, more long-standing, always internally generated
