NURS 7053 ALTERATIONS OF CARDIOVASCULAR FUNCTION IV Right Ventricular Failure Learning Outcomes By the end of this module, learners will be able to: 1.Define the key characteristics of right ventricular failure and how it differs from left ventricular failure. 2.Describe the common etiologies of right ventricular failure. 3.Analyze the pathophysiological mechanisms underlying right ventricular failure. 4.Explain how the pathophysiology of right ventricular failure leads to specific clinical manifestations. Alt CV Fcn IV Learning Activity1: Diagram the Heart Alt CV Fcn IV RIGHT VENTRICULAR FAILURE Etiologies Right ventricular myocardial ischemia/infarction (MI) RCA Right Ventricular Failure Etiologies Left ventricular failure – Pulmonary hypertension (HTN). – Increase in RV afterload. Pulmonary HTN LVF Arrows are showing the back up of pressures caused by the failure of the heart to pump the blood forward. Right Ventricular Failure Etiologies Primary pulmonary hypertension – Rare condition – Chronic in nature, so we see right ventricular hypertrophy. PPH RVF We’re not going to focus on the patho of PPH. If you’re interested, you can read more about it on p. 1102 in the text. Right Ventricular Failure Active Learning Break 2: Flowchart Etiologies Secondary pulmonary hypertension – Pulmonary vascular obstruction Blood clot RVF Right Ventricular Failure Etiologies Pulmonary Disease – Chronic pulmonary diseases such as COPD and cystic fibrosis eventually leads to RVF. Right Ventricular Failure RV hypertrophy from pulmonary disease Back up of pressures into lower limb veins Increased capillary pressures EDEMA Learning Activity 2: Flowchart Right Ventricular Failure FYI - Clinical Manifestations Elevated CVP & jugular vein distention Heart with Sawn-Ganz catheter in place for hemodynamic monitoring Ext. jugular vein Sternocleidomastoid From: ADAM Interactive Anatomy Right Ventricular Failure FYI - Clinical Manifestations Peripheral edema with weight gain High peripheral capillary pressures Back up of pressures into lower limb veins EDEMA Right Ventricular Failure FYI - Clinical Manifestations Hepatosplenomegaly Inferior vena cava Hepatic portal vein Splenic vein Inferior mesenteric vein Superior mesenteric vein Veins of the digestive system that feed into the hepatic portal vein Right Ventricular Failure FYI - Clinical Manifestations Manifestations of decreased CO from LV Hypotension & tachycardia Decreased urine output Pale cool skin & mottling Decreased CO Right Ventricular Failure Learning Activity 3: Create a Patient Case
