Colostomy Definition: Colostomy: is a surgical procedure in which the colon is shortened to remove a damaged part and the cut end diverted to an opening in the abdominal wall. It may be temporary or permanent. Purpose of colostomy : ​ It is applied when feces cannot progress naturally from the colon to the anus ​ It is performed in any condition where the rectum or anus is nonfunctional because of disease, a birth defect or a traumatic condition. ​ To divert the fecal flow away from an area of inflammation or around an operative area. ​ To treat various disorders of the large intestine, including: cancer, obstruction, inflammatory bowel disease, ruptured diverticulum, ischemia (compromised blood supply), or traumatic injury. Indication: ​ Birth defect. ​ Inflammatory bowel disease. ​ Injury to the colon or rectum. ​ Partial or complete intestinal or bowel blockage. ​ Rectal or colon cancer. Criteria of healthy stoma: ​ 1 cm above skin level ​ Moist ​ Red / pink color ​ Painless ​ No evidence of erythema, rash, ulceration or inflammation in the surrounding skin. Classification: According to duration: A. Permanent Colostomy B. Temporary Colostomy According to stoma site: A. Ascending Colostomy B. Transverse Colostomy 1-Loop transverse 2- double-barrel transverse C- Descending Colostomy Assessment of stoma: Stoma colour: Normal---- pink / red / warm to touch Abnormal –--- black / dusky / pale /sloughy Skin : Normal---- in the post-operative period the stoma can be quite swollen. It may reduce in size for about 6 weeks after surgery. Abnormal---- Any sudden or unexplained swelling of the stoma. Bleeding stoma: Normal---- A slight smear of blood on the wipe when washing or drying the stoma. Abnormal---- Excessive bleeding when cleaning the stoma/blood in the pouch / bleeding from inside the stoma. Complications: ​ Leakage. ​ Prolapse ​ Obstruction or stenosis. ​ Stoma becomes edematous and enlarged.
