Academic Rating 20% Final grade in the learning area or average of the final grades in subjects specifically related to the award. (MOV’s: Photocopy of the final grades signed by the adviser/subject teacher/s.) 2. Skill in the Discipline 40% As shown through: a. Output ( oral or written work, projects, etc., if applicable ) (10%) (MOV’s: Project proposals, documentation of projects, and other works) b. Membership in a club/team ( if applicable ) (10%) (MOV’s: Certificate showing that the candidate is a member or officer of the club/team related to the discipline.) c. Class or school representation (10%) (MOV’s: Documents showing class and/or school representations.) d. Winnings and awards (10%) (MOV’s: Photocopies and/or images of certificates and/or awards, medals and the like.) 3. Attitude toward the Discipline 20% a. Peer evaluation ( if applicable )** (MOV’s: Score sheet indicating the score of the candidate.) b. Commendation from coach/adviser (MOV’s: Certificate of Commendation form the coach/adviser.) 4. Contribution to the School related to the Discipline 20% In any of the following: a. Tutorials/ Coaching b. Performance in school’s various functions and events c. Products d. Projects e. Volunteer work The candidate shall present any or, if applicable, all documents supporting the above criteria that shows his/her contribution to the school related to the Discipline. TOTAL RATING **Peer evaluation: The 10% will be coming from the peer assessment that will be separately conducted by the awards committee.
