Aspiration Policy Summary & Quiz Subject: Aspiration Precautions Policy: Woodside Manor Nursing Home will maintain the wellbeing of residents on aspiration precautions while meeting socialization needs and allowing the resident’s to eat in a timely manner. Procedure: If aspiration precautions are indicated on transfer orders, the orders will be transcribed to the admission orders by the nurse completing the admission. An order will also be obtained for a swallow evaluation to be completed by a speech pathologist in house. If a recommendation for aspiration precautions is designated by a speech pathologist in house, an order will be obtained from the medical provider and the consulting dietitian will be notified. Reminders: Aspiration risk residents are to be in view of a nursing staff member for all meals. The resident may be in the high observation areas at the nurses’ station or the dining room. The head of the beds for residents under aspiration precautions should never be flat. The resident under aspiration precautions must be upright for all meals and remain upright for 30 minutes following the completion of eating. A resident on aspiration precautions must be monitored by a nurse until all oral intake including fluids, food, and medications are administered to ensure safe swallowing without choking. A nurse is required to be present in order to promptly intervene in the event of choking, necessitating abdominal thrusts, suctioning, or CPR from the nurse. Aspiration precautions must be indicated on the medical orders, the dietary card (meal ticket), and on the Care Cards at the nurses’ station. A yellow dot can be utilized to mark a resident as an aspiration risk. The yellow dot may be applied to care cards, room numbers, and or resident IDs. If a resident is coughing while ingesting their current ordered diet or consistency of fluid the nurse may downgrade the diet or consistency of fluid within nursing judgment. A progress note must be documented within the EHR, complete with vital signs. Notification to the medical provider, speech pathologist, and dietary department must occur. The dietary adjustment and provider notification must be recorded in the Provider Logbook for follow-up assessment when the provider is onsite. The resident’s adjusted diet or fluid consistency must be maintained until accurate recommendations are received from the medical provider and speech pathologist. The resident’s subsequent dietary adjustment must be communicated and recorded within the 24 hour report.
