**Extra Oral Examination- Self assessment questions** ***Q1 What's the purpose of an extra oral examination?*** *A: To identify any undisclosed or undiagnosed conditions, which may include malignancies.* **Q2 Which areas should you be looking at during an EO exam and why is this important?** A: [Upper Face]: Forehead, Eyes, Cheeks/cheekbones, Upper part of nose [Lower Face:] Lower part of nose[,] Lips[,] Chin[,] General symmetry Looking for significant asymmetry- may have an underlying cause- need to check history with patient. Any unexplained lesions- patient will not be able to see any lesions on side of face which may be malignancy; any bruising/swelling/excessive sweating- may be indicator of underlying systemic disease? Anything on lips/corners of mouth- may be herpes -contagious- **cannot treat!** May be angular cheilitis- fungal infection. Squamous cell carcinoma may present as ulcer/blister/red/white patch **Q3 What will you generally be looking for during the EO exam and how will you do this?** A: Any swelling, tenderness, abnormality We need to **PALPATE** (feel) for swelling, tenderness and lymph gland enlargement **Lymphadenopathy:** Disease of the Lymph glands, often recognised as swelling and tenderness **Q4 What's the best operator position for carrying out a lymph node/TMJ examination?** A: Stand behind patient; Ensure patient is relaxed; Their head should be slightly forward, make sure chin is not tipped upward; **u**se the pads of the fingertips in an on/off motion, Examine all nodes. Examine both sides of TMJ at the same time with tips of fingers and gentle pressure. **Q5 Name and explain the location of the lymph nodes of the head and neck** Preauricular: Anterior to the earlobe; postauricular: (posterior to the ear); Occipital (posterior to the postauricular); parotid (region of gland); retropharyngeal-(tonsillar region); submandibular- (posterior area); submental- (anterior area); anterior cervical & posterior cervical; supraclavicular --(superior region of shoulder) **Q6 What should healthy lymph nodes feel like?** A: should feel small (pea size) and not tender to touch. Also usually mobile. **Q7 How should you document any lymph node abnormality is found?** A: make careful notes on size, exact location, consistency, history if any, use diagrams **Q8 What should feel for when you carry out a TMJ examination?** A. Tenderness, pain, deviation to one side, clicking, limited opening **Q9 What questions can you ask the patient if you identify any abnormality extra orally/intra orally?** A: Should ALWAYS review medical history- any acute/chronic/systemic/oral conditions? Depending on findings, can ask the following questions to the patient: How long has it been there? How are you feeling in yourself? Are you experiencing any pain? How often do you get ulcers? Do you remember biting your tongue/lip? Do you remember eating anything very hot and burning yourself? Are you aware of clenching/grinding your teeth?
