Volume: Chapter: Originated By: Subject: Date: Review Month: 1 ADMINISTRATIVE POLICES Emergency Operations Manual – Article 1/Section 6 Emergency Response Division (Safety Officer) Post Incident Analysis (PIA) 3/15/2004 (Edited: 12/23/08) Open 01.01 PURPOSE: To review departmental operations at the scene of emergencies by evaluating personnel actions and operational procedures employed in mitigating the emergency. 01.02 REFERENCES: NFPA 1500, Standard on Fire Department Occupational Safety and Health Program, 6-8 Incident Critique. 01.03 POLICY: It shall be the policy of the Department of Fire-Rescue to analyze and review the actions taken by personnel at the emergency scene or other significant incident or activity. This basic review shall include the conditions present, actions taken, and the effect of the conditions and actions on the safety and health of members. The Post Incident Analysis (PIA) Summary Sheet shall be used for this purpose. It is required on all Code 1 incidents (except single family dwellings), other significant events such as Hazardous Materials, Technical Rescue or Dive Rescue responses, when more units are utilized on the scene than originally dispatched, or when directed by the Assistant Fire Chief of the Emergency Response Division (ERD). 01.04 RESPONSIBILITY: The Incident Commander shall complete the Post Incident Analysis (PIA) Summary Sheet and forward to the Assistant Fire Chief of ERD. Company officers, division or group supervisors, and members shall assist the Incident Commander by providing all pertinent details pertaining to their assignment and actions at the incident. 01.05 PROCEDURE: I. POST INCIDENT ANALYSIS (PIA) SUMMARY SHEET A. FORM COMPLETION 1. The Incident Commander shall complete all applicable sections of the Post. Incident Analysis Summary Sheet after interviews and personal observation of the incident. 2. Forward completed summary to the Assistant Fire Chief of ERD. II. DISTRIBUTION OF SUMMARY 1. The Assistant Fire Chief of ERD shall distribute information to all members as needed for review. Edited: 12/23/08
