Duty Related Death, Line Of Duty Serious Injury And Funeral Protocol Manual 1 Duty Related Death, Line Of Duty Serious Injury And Funeral Protocol Manual DUTY RELATED DEATH, LINE OF DUTY SERIOUS INJURY AND FUNERAL PROTOCOL MANUAL BOOK 88 May 14, 2009 TO: All Civilian/Uniform Members SUBJECT: DEPARTMENT DUTY RELATED DEATH, LINE OF DUTY SERIOUS INJURY AND FUNERAL PROTOCOL MANUAL The line-of-duty death or serious injury of a Department member, both uniformed and non-uniformed, generates grief beyond measure for family members, the Department and the community. The Department has various obligations in these difficult situations that may include ensuring the highest level of medical care to commemorating the individual, comforting family members and exhibiting the time-honored traditions of the Fire Service. Likewise, the death of a firefighter/or civilian member retiree or close friends of the Fire Department can also prompt a response where a commitment of Fire resources might be appropriate. The Duty Related Death, Line of Duty Serious Injury and Funeral Protocol Manual, which replaces the Funeral Manual, is designed to support and facilitate the Department’s response to these events. By providing a comprehensive preplanned LODD/SI Emergency Response Team format and funeral procedure, it is hoped that the the Department will be able to support the well being of involved members, render comfort to the family and, when necessary, allow the Department and the community to bid a dignified farewell to a fallen comrade. It is my hope and prayer that we will never need this book. Stay safe, Brian Cummings Fire Chief SECTION I INTRODUCTION PURPOSE The intent of this manual is to provide a comprehensive guide for the mangement of a Line of Duty Death/Serious Injury (LODD/SI) event and, when necessary, a protocol checklist for memorial events and services. Whether a death is expected or sudden, the conditions that prevail will require specific alignments in the organization as the desires of the family and direction of the Department are clarified. The Department will carefully consider any resource request for a Level I – VI Funeral that occurs in the Southern California Region. The desires of the family will be accommodated within Department guidelines and policies throughout the decision-making process. The Fire Department's responsibility will include working closely with a funeral director in order to accomplish the desires of the Fallen Member's family. The specifics include: A. Proper organization and coordination B. Good communications C. Maintenance of flexibility D. Distribution of workload and responsibility E. Moral and emotional support to the member’s family F. Meeting the requests and needs of the surviving family members and Department Duty Related Death, Line Of Duty Serious Injury And Funeral Protocol Manual 2 This manual will help to meet these needs by providing Department guidelines and policy options that the Line of Duty Death/Serious Injury Assignment Coordinator and Department’s Funeral Commander may follow in whole or in part. It is important to ensure that all Fire Department personnel involved in the management of the line of duty death/serious injury, and memorial events and or services understand their roles. CONFIDENTIALITY Given the serious nature of a line of duty death or serious injury, and our deep regard for the emotional well-being of family and surviving members, all information acquired during performance of LODD/SI ERT duties is considered confidential. Event information shall not be released to non LODD/SI ERT team members without first receiving permission of the Assignment Coordinator. This statement is not meant to preclude sharing of information with those entites performing required tasks such as LODD/SI ERT activities, law enforcement action, and or SIIT invesitgations. TEAM ACTIVATION The Department will activate the Line of Duty Death / Serious Injury Emergency Response Team whenever any LAFD member is critically injured or killed in the line of duty. The Fire Chief and or Deputy Department Commander may activate the LODD/SI ERT for other incidents at their discretion. STAFFING The Team consists of a cadre of trained members who are on call for immediate assignment to facilitate efficient, compassionate and humane management of all serious injury and line of duty death incidents. Activated members shall be guided by the Assignment Coordinator who functions as the Team Leader. TRAINING All team members shall participate in training developed and or designated by the Department as pertinent. GLOSSARY ADMINISTRATIVE OFFICE The Office of the Fire Chief. ASSIGNMENT COORDINATOR The Planning Section Commander or his/her designee serves as the Assignment Coordinator of the Line of Duty Death/Serious Injury Emergency Response Team in this position. BADGE SHROUDS Black memorial bands, to be worn horizontally across the badge, may be worn due to line-of-duty deaths within the LAFD or LAPD and as directed by the Fire Chief. Badge covers may be worn from the time of death until the conclusion of the funeral. Badge covers are available from the Firemen’s Relief Association. BAGPIPES Used in traditional fire service ceremony. “Pipers” accompany and play music during movement of the casket and, if desired, during the service. BELL CEREMONY AND PRAYER The ringing of the bell and the Fire Fighter’s Prayer are two traditions of the fire service which reflect respect and honor to those who gave their lives to their duty. The ringing of the bell represents the end of the emergency and the return to quarters. Within the Los Angeles Fire Department, the bell is rung ten times in solemn succession. Duty Related Death, Line Of Duty Serious Injury And Funeral Protocol Manual 3 BEHAVIORAL HEALTH AND WELLNESS Formerly Critical Incident Stress Management (CISM) BEREAVEMENT OFFICER The Department Chaplain serves as the Line of Duty Death/Serious Injury Emergency Response Team Bereavement Officer. The Bereavement Officer works in conjunction with the Family Notification Group and Funeral Command Team. Clergy of a variety of faiths are prepared to assist when called upon. CHAPLAIN The involvement of the Department Chaplain in the funeral process will depend upon the family’s wishes and/or religion. Currently, the Department has Chaplain representatives for the following faiths: Catholicism, Christianity, Islam, and Judaism. Also, the Chaplain serves as the Bereavement Officer on the LODD/SI Emergency Response Team. CHIEF OFFICER IN CHARGE OF FUNERAL DETAIL Depending upon the type of funeral, the officer in command should consult with those in charge of the services including the Planning Section and/or a representative of the Relief Association, and conduct the detail in accordance with existing conditions and circumstances. COLOR GUARD A ceremonial escort for regimental display of the “Colors.” Standard flag presentation shall include three flags: the American Flag, the flag of the City of Los Angeles, and the flag of the Los Angeles Fire Department. COMMUNICATIONS One person shall have the primary responsibility of keeping Department members informed regarding service arrangements, funeral details, transportation arrangements, half-staff flag information, staging locations, etc. EULOGY Fire Service member/s speaking in remembrance as a part of the funeral service. FAMILY LIAISON OFFICER (FLO)The member of the Line of Duty Death/Serious Injury Emergency Response Team that serves as a link between the family and the Department. The FLO provides assistance to the family/survivor from initial event notification through the funeral process and distribution of survivor benefits. This position is typically a Planning Section Captain as designated by the Assignment Coordinator. The FLO is on-call 24 hours per day. FAMILY / SURVIVOR NOTIFICATION GROUP The Fire Chief or Deputy Department Commander, Family Liaison Officer, Pre-designated Contact, and Line of Duty Death/Serious Injury Emergency Response Team Bereavement Officer comprise the team responsible for Family/Survivor notification. FIRE ENGINE CAISSON A Fire Engine Caisson is the fire department pumper, which carries the casket. FLY-BY MISSING MEMBER FORMATION This is a traditional formation of helicopters flown during a designated time of a funeral ceremony. One helicopter exits from the formation to signify a lost member. This has been done at various times during a service to include the moment when the fallen firefighter is moved from the procession into the church or at the conclusion of the graveside interment. Duty Related Death, Line Of Duty Serious Injury And Funeral Protocol Manual 4 FUNERAL COMMANDER The Department member with overall responsibility for all events and services related to a Fallen Member’s memorial. FUNERAL DETAIL Composed of volunteers requested by teletype notice. The family of the deceased may request a funeral detail through the Relief Association. Once the family asks for a detail, the Relief Association will request a teletype notice through the Administrative Office seeking volunteers. Members of the detail shall report to the officer in charge at least 30 minutes prior to the time of the service. Members of the detail shall be in dress uniform. Off-duty members not a part of the detail attending the funeral services of an active or retired member may wear the dress uniform. HONOR GUARD The delegation of Department representatives that handles, moves, and guards the casket of the Fallen Member, in the highest tradition of respect. The suggested number of Honor Guards is six. Type I Funerals will have a standard deployment of a full Honor Guard. In Type II, III, and IV Funerals, a request for an Honor Guard will be made to the Los Angeles Firemen’s Relief Association and formed from a volunteer list gathered by a teletype notice. When the member’s family selects a ceremony that includes a "viewing period" for the deceased; they might wish to have an Honor Guard standby with the deceased during this period of viewing. Two members at a time should guard the casket. HONORARY PALLBEARERS As an option to a traditional Honor Guard, six to eight persons designated by the member’s family (with Department participation offered), may act as Pallbearers for the deceased. The family may designate Honorary Pallbearers. This option is generally exercised at funerals of VIP's or dignitaries. IMPACTED COMPANY LIAISON (ICL) The member of the Line of Duty Death/Serious Injury Emergency Response Team responsible for the well-being of on-scene impacted companies. The Impacted Company Liaison works in conjunction with Behavioral Health and Wellness, SIIT, Incident Commander, and others to facilitate the efficient, compassionate and humane implementation of required procedures. LOS ANGELES FIREMEN’S RELIEF ASSOCIATION (LAFRA) Los Angeles Firemen’s Relief Association offers programs to provide assistance to the firefighters and their families. Such programs include the Fire Medical Plan, Relief Fund, Life & Accident Fund, and the Widows, Orphans & Disabled Firemen’s Fund. LINE OF DUTY DEATH/SERIOUS INJURY EMERGENCY RESPONSE TEAM (LODD/SI ERT) A multi-disciplined team responsible for the efficient and compassionate management of the line of duty death and or serious injury from initial event through the distribution of survivor’s benefit. MEDICAL LIAISON UNIT (MLU) Administers Department policy regarding medical leaves. Representing all Members of the Department in matters related to injury or illness. MEDICAL SITE OFFICE The EMS Battalion Captain (PM cert) of the Line of Duty Death/Serious Injury Emergency Response Team responsible for overseeing initial medical care of the seriously injured and required medical and or autopsy procedures for fatally wounded members. The Medical Site Officer will work in conjunction with the on-call Medical Liaison Officer and Department Medical Advisor. NOTIFICATION OFFICER Duty Related Death, Line Of Duty Serious Injury And Funeral Protocol Manual 5 A procedure used by the Administrative Office (Community Liaison Officer) to simplify and disburse information to other departments or agencies interested in attending and showing respect to the Fallen Member. PERSONAL BELONGINGS OFFICER A Chief Officer of the Line of Duty Death/Serious Injury Emergency Response Team responsible for the collection and management of the injured/deceased member’s personal belongings. PRE-DESIGNATED FAMILY CONTACT The Member identified on the “Next of Kin” or “Pre-designated Family Contact” forms as the person that should assist with Family/Survivor notification. PROCESSION OFFICER Coordinates the transportation arrangements and funeral procession from the funeral home to the church and to the cemetery. PUBLIC SAFETY OFFICERS’ BENEFIT PROGRAM (PSOB) The PSOB Program provides a one-time benefit to the eligible survivors of public safety officers whose deaths, or permannent and total disability, are the direct and proximate result of a traumatic injury sustained in the line of duty. SIGNIFICANT INCIDENT INVESTIGATION TEAM (SIIT) The team of individuals designated by the Department to participate in the SIIT process.The overal puprpose of the SIIT process is to determine the cause and contributing factors of work related deaths and severe injuries among Firefighters and Paramedics. STATIC EQUIPMENT DISPLAY Apparatus with assigned members are stationed along the procession route in front of Fire Stations and locations as appropriate and designated by the Funeral Commander (e.g., crossed aerial display). Uniformed members shall wear dress uniform, in line at a position of attention. All members will tender the hand salute. TYPES OF FUNERALS There are six (VI) circumstances under which a Department Member’s, or select non-Department member’s, death occurs which may prompt specific funeral response: I. Active Department Member Line-of-Duty-Death (LODD, IOD): An active member is killed during an incident, or dies as the direct result of injuries sustained during an incident. II. Active Department Member Indirect Line-of-Duty Death (IOD): An active member dies during the course of employment, and the death is ruled to be the result of a line-of-duty cause but is not directly incident-related. III. Retired Member Cause is Duty Connected (IOD): A member dies as a result of an IOD cause. IV. A Member (Active or Retired) Cause is Non-Duty Related: Death is ruled to be not Line-of-Duty. V. A Member of the Fire Commission or dignitary: Individual typically has a close association with the Department and the Fire Chief wishes to bestow honors. VI. A member of another fire agency: LAPD or other Law Enforcement Agency expires and the Department wishes to bestow honors. UNIFORM DETAIL A delegation of uniformed members (normally a minimum of six members and one Chief Officer) assembled as an organized delegation to represent the Department and honor the Fallen Member. UNIFORMED MEMBER Duty Related Death, Line Of Duty Serious Injury And Funeral Protocol Manual 6 For purposes of this manual, “Uniformed Member” includes L.A.F.D. sworn members wearing the Department uniform. PROCEDURAL GUIDELINES FOR EACH TYPE OF FIRE DEPARTMENT FUNERAL TYPE I: ACTIVE DEPARTMENT MEMBER LINE-OF-DUTY-DEATH (LODD, IOD) 1.Implementation of the Funeral Incident Command System under the command of the Funeral Incident Commander (FIC). 2.LODD/SI ERT Bereavement Officer (Department Chaplain), Los Angeles Firemen’s Relief Association and labor organizations (United Firefighters of Los Angeles City & Chief Officer’s Association) notifications are made in order to provide support and assistance to the Fallen Member’s family. 3.Apparatus and personnel from the Fallen Member’s Fire Station (and additional companies as designated) placed out of service to attend funeral. 4.Static display of apparatus at designated locations along the procession route (e.g., in front of fire stations), with members in dress uniform. All members will stand at attention and tender the hand salute as the procession and casket passes. 5.A helicopter fly-over in the "missing member" formation is made during the designated stage of the memorial service. 6.The American Flag at the member's last place of assignment is retired from duty and presented to the appropriate survivor by the Fire Chief or a designated representative. TYPE II: ACTIVE DEPARTMENT MEMBER INDIRECT (DUTY CONNECTED) 1.A Chief Officer is assigned to take charge of the funeral detail. 2.Community Liaison Office notification. 3.Apparatus and personnel from the Fallen Member’s Fire Station (and additional companies as designated) placed out of service to attend funeral (see Resource Commitments, Type II). 4.Static display of apparatus at designated locations along the procession route (e.g., in front of fire stations), with members in dress uniform. In City Battalion Chief 50 miles. TYPE V: A MEMBER OF THE FIRE COMMISSION, DIGNITARY, OR INDIVIDUAL DEEMED APPROPRIATE BY THE FIRE CHIEF 1.At the Fire Chief’s discretion, a Department Chaplain is designated to offer solace to the family. 2.A Chief Officer and funeral detail may be assigned. 3.Honorary Pallbearers may be assigned according to the family's wishes. 4.Utilization of Department apparatus (see Resource Commitments, Type V). The Commanding Officer will tender the hand salute as the hearse passes. 3.For funerals that are conducted outside of the City, a static display of apparatus at the funeral entrance or appropriate alternate location(s).
