LGBTI and Nursing Approach in Disadvantaged Groups THE EVENT WHERE THE PHYSIOLOGICAL AND PSYCHOLOGICAL RELATIONS ARE MOST CLEAR IS SEXUAL ERİCH FROMM Sexuality is a concept that starts before birth and continues throughout life, shaped by people's values, beliefs, feelings, personalities, likes and dislikes, attitudes, behaviors, physical appearance and the societies they live in. It includes the entire body and mind, not just the genitals. Sexual health is an integral part of human health. sexual health; It is a state of complete physical, emotional and social well-being in terms of sexuality and is a fundamental right for all people, men and women, young and old. Sexual health is the ability of a person to continue their sexual life without any coercion, happily and without harm. The World Health Organization (WHO) defines healthy sexuality as “a positive integration and enrichment of the somatic, emotional, intellectual and social components of personality, communication and love. defined as “integrity that develops”. Sex: It includes the physiological characteristics that determine a person as male or female. Gender: Expresses widely accepted ideas and expectations (norms) for women and men. Gender identity: It is how individuals define themselves in their inner world. It is the personal and private identity one has by being a woman or a man. Sexual orientation: It is a characteristic defined according to the gender to which the person is sexually attracted to thoughts, feelings and behaviors, as grouped below: Interest in the opposite sex (Heterosexual): A person's sexual interest in the opposite sex Same-sex interest (homosexual, homosexual): Having sexual interest in one's own sex, Being bisexual: Having sexual interest in people of both sexes. Being transgender: She needs to change her gender, mentally and physically believing that they should have the other gender Self-respect, trust- Mutual trust and respect Avoiding sexual abuse of self and others Being honest with yourself and others Knowing the rights, responsibilities and duties regarding sexual intercourse To be understanding and caring towards individuals with different views and understanding To be tolerant and understanding of differences in religion, culture, sexual preference and orientation Developing sexual self-discipline Negative or inappropriate attitudes of the family to sexuality Deteriorated family relationships Don't be a perfectionist organic diseases Seeing sexuality as a tool for success and performance traumatic sexual experience Low self-esteem of men and women Communication disorder between spouses Jealousy Although Lesbian, Gay, Bisexual, Transgender and Intersexual (LGBTI/LGBT+) individuals have become more visible in recent years, they are still among the risky groups that are excluded and stigmatized due to homophobia. Some factors that can reinforce homophobia on a larger scale are the moral, religious, and political beliefs of a dominant group. In some countries, homosexuality is considered illegal and homosexuals are punished with fines, life imprisonment or even the death penalty. There is no expression of "sexual orientation" in the Turkish Civil Code and there is no penal clause against homosexuality. In a study conducted with LGBT individuals in Turkey, it was reported that discrimination against LGBT individuals is high, this situation significantly affects their daily lives, and negative attitudes towards homosexuality are displayed due to the general perspective of the society. Although it was previously considered a crime in the Turkish Republic of Northern Cyprus; In the law that has been going on since the British colonial period; The relevant articles of the Penal Code (Chapter 154, 171-173), which regulates the relationship between two men at the age of consent as a crime against "nature", were repealed on 27 January 2014 and decriminalized. In TRNC It has been reported that LGBTI+ individuals do not see themselves as different from other individuals around them, but they hesitate to express their sexual orientation openly. It is reported that LGBT individuals are exposed to intolerance, discrimination, harassment and threats of violence more than those who identify as heterosexual because of their sexual orientation. Such approaches may emerge more with the increase in the sense of homophobia in the society. Depending on this fact, they are exposed to many inequalities in the field of health as well as in all areas of their lives. Sexually oriented people experience inequality in Healthcare services, largely related to their experience of discrimination and homophobia. These distinctions lead to avoidance of routine health care and screening services. This is linked to increased rates of smoking, alcohol use, depression, HIV/AIDS, physical violence and suicide attempts among LGBT individuals. The Universal Declaration of Human Rights states that every human being has equal rights from birth, and health is one of these rights. In the studies conducted, it is stated that LGBTI individuals are exposed to discrimination and bullying, they are excluded, they encounter negative attitudes and behaviors, even they cannot receive health care services, and they avoid getting health services due to negative experiences. With these inequalities faced by LGBT individuals who cannot access health services, encounter wrong attitudes and behaviors, and have problems with accessing health information, their health status may worsen and individuals who experience such difficulties cannot protect both their physical and mental health. The knowledge, attitudes and skills of health professionals are extremely important in preventing health inequalities experienced by LGBTI individuals at risk. With these inequalities faced by LGBTI individuals who cannot access health services, encounter wrong attitudes and behaviors, and have problems with accessing health information, their health status may worsen. Individuals who experience such difficulties cannot protect both their physical and mental health. Despite the fact that the right to receive health care, which is one of the basic human rights, is accepted as a service guaranteed by the state (Patient Rights Regulation, 2016), prejudiced, homophobic attitudes, heterosexist practices, trust problems and previous negative experiences, especially the lack of knowledge and experience of health professionals. There are many problems in receiving health care services for LGBTI individuals depending on their experiences. Approach of health professionals; The knowledge, attitudes and skills of health professionals are extremely important in preventing health inequalities experienced by LGBTI individuals. The role of nurses, who are in contact with patients the most, is becoming undeniably important. Approach of health professionals; Elimination of knowledge deficiencies of health professionals and especially nurses; Eliminating the lack of knowledge and experience in order to provide individuals with the quality and fair health care they deserve, and the inclusion of LGBTI and health problems in the nursing curriculum are seen as important issues.
