Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) Military Training Instructor (MTI) DAILY ACTIONS Ensure flight accountability throughout the day. Review BMT Form 118, Entry Controller Checklist, for accuracy and completeness. Note: Identify errors with red checkmarks; ensure two BMT Form 118’s (dated front and back) are on the EC stand or in the EC binder. (MTI must inspect the BMT Form 118 daily (NLT 0900 summer and 0800 winter schedule). Inspect hotline phones, lighting, plumbing, windows, doors, stairwell, and empty lockers for unauthorized items and dorm furnishings for possible defects. Tag (discrepancy, date, and MTIs name) and report discrepancies that you cannot fix to the Physical Training (PT) Supply NCO. Ensure Civil Engineering (CE) work orders are properly logged, tracked, and completed, the work order number will be added to the tag within three days of reporting the discrepancy to the PT/Supply NCO. If hotline phone is inoperable, notify CQ for documentation in the CQ blotter and leadership action. Conduct Airman’s Time: Topic for discussion will be IAW Weekly Activity Schedule and Plan of Instruction. Conduct evening briefing and include as a minimum: ▪ Customs and courtesies. ▪ Core Values. ▪ Next day’s activities and requirements (to include details and dorm departure times). ▪ Evening schedule. ▪ Mail call - Mail pick-up is 1030 to 1700 hours (first-come, first-serve) through Friday, 6WOT. ▪ Accountability Check. ▪ Obtain medical waivers. ▪ Hydration and personal hygiene requirements and hand sanitizing. ▪ Discuss wingman policy (adherence to requirements and reporting if any Trainee was separated from their wingman). ▪ Remind Trainees there is a no-tolerance policy for being up after 2100 hours (lights-out) unless Trainees are returning late from scheduled details, late processing appointments, demand reduction, entry controller (EC) duty or latrine use. Ensure Trainees identified on the “Red Medical Alert (RMA) ID Tag Report” wear the RMA ID Tag in all uniform combinations during all activities. NOTE: Trainee’s will continue to wear the ID tag in Tech Training. MTIs are required to have the flight Clip or iPad in their possession at all times when leading a flight (exception: MTIs assigned to PACER FORGE and during graduation events). At the end of the day, the Clip or iPad may be left in the Flight Commander (FC) or Instructor Supervisor (IS) office. The Clip or iPad will include the following mandatory items digitally or on person at all times: ▪ Weekly Activities Schedule (WAS). ▪ Corrected Flight Roster and Entry Authorization List (EAL). ▪ AETC Form 50, Appointment Slips. ▪ BMT Form 107, Record of Drill Progress. 1 Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) ▪ Black/Blue Ink Pen (or on person). ▪ Heat Stress Prevention Card (hard copy or electronic). ▪ Training Weapons ID Roster (upon issue and until weapons turn-in). ▪ Red Medical Alert ID Tag Roster. Ensure flight performs daily details IAW lesson plans. Flights fall out daily Monday-Saturday for reveille and accountability. Flights must be in the UOD for assembly/PT but flight instructor may apply good judgement to upgrade/downgrade throughout the day but will remain uniform as a flight. Airman's Creed: at a minimum the flight MTI must have the entire flight recite the Airman’s Creed once in the morning. Flight MTIs provide Trainees a minimum of five hours study time per week IAW WAS. NOTE: Trainees may only miss classes for medical and mandatory processing appointments. However, every effort should be made to preclude Trainees from missing military studies. The flight MTI ensures Trainee receives required information from the academic monitor. Classroom Procedures The flight MTI will ensure: ▪ Trainees have a latrine break within 20 minutes of arrival to classes. ▪ A current EAL/Flight Roster (No SSNs) is provided to the class instructor. ▪ Trainees do not enter classrooms without class instructor knowledge, to ensure adequate supervision. Flights arrive in class 10 minutes prior to the class start time. If classes are located at a DCF: When arriving to class (this includes returning to class after lunch) flight MTIs will escort their flight upstairs to transfer responsibility and rosters to the class Instructor. Place the Guidon Bearer (GOB) in the front, the Dorm Chief in the back, and Trainees who have appointments (with their wingman) directly in front of the Dorm Chief. Following each class, four Trainees will remain behind to clean the classroom with disinfecting solution. If classes are located at a RH&T: After ensuring the flight is properly sized, the flight MTI will place Trainees who have appointments during class in the back of the fourth element (in front of the Dorm Chief) with their wingman then give their flight “Column of Files from the Left.” Following each class, four Trainees will remain behind to clean the classroom with disinfecting solution. Bob Hope Theater: Trainees will enter and exit through the front of the building. Following each class, all Trainees will sanitize the auditorium and latrines with disinfecting solution. If the Trainee does not return with an appointment slip(s) and/or does not get credit for the class, the flight MTI will report the discrepancy to their IS. The IS will e-mail War Skills Military Study at 737 [email protected] with the flight number, name of Trainee and roster number, name of the class, and the date the class was held. If the Trainee or Trainees in attendance do not get credit for a graduation required class after 24 hours, the flight MTI will report the discrepancy to their IS. The IS will e-mail WSMS at 737 TRSS.WSMS/Missing/[email protected] with the flight number, name of Trainee and roster number, name of the class, and the date the class was held. AAFES (BX Troop Stores) Flight visits to AAFES troop stores require coordination by FC/SC with TRG Scheduling. BEFORE ARRIVAL OF THE FLIGHT ZERO WEEK SQUADRON & GAINING SQUADRON Ensure the dorm is clean and sanitized. Check dorm lighting, electrical outlets, soap, paper towel dispensers, plumbing, air conditioning, and water station filter. Procure flight pickup items (e.g., road guard vests, civilian luggage tags, etc.). Ensure each bed is properly made. Restock the following forms: ▪ AETC Form 50, Appointment Slip. ▪ AETC Form 341, Excellence/Discrepancy Report. ▪ BMT Form 106, Entry Controller Roster. ▪ BMT Form 107, Record of Drill Progress. ▪ BMT Form 118, Entry Controller Checklist. Ensure access board, bulletin boards, and mailing address, correctly posted. Inspect flight guidon for serviceability and report any discrepancies to squadron Military Drill and Ceremony (MDC) instructor. Inspect dorm hotline phones for operating serviceability. Ensure processing areas are setup the night prior to flight arrival for agencies processing on Wednesday thru Friday. Following processing, centrally place all tables/chairs for pick-up on Monday, 1WOT. Ensure Trainee iPads are charged, connected to wifi and up to date on software. FLIGHT ALERT ZERO WEEK SQUADRON 0WOT squadron will transit Trainees from PRC to the 0WOT squadron after they have been processed on Tuesday, Wednesday and Thursday. PRC Staff will inventory items and contact 0WOT squadron with known missing items (i.e.; chains, watch caps) with a projected time of arrival of items. Missing backpack items (listing provided inside backpack) must be provided to AAFES directly by each Trainee. -------------------------------- Sat 0WOT – Flights transit to gaining squadron. Conduct Line Squadron Orientation/Bed Down IAW WAS. 3 Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) ZERO WEEK SQUADRON IMMEDIATELY UPON ARRIVAL OF THE FLIGHT FLIGHT PICK-UP/BED DOWN ORIENTATION CHECKLIST Downstairs Take roll call/accountability. Teach reporting statement/position of attention. Brief Trainees on hygiene. Emergency Evacuation Procedures. In the Bays Assign Trainees to a Personal Living Area (PLA – wall locker and bed). Have Trainees find their beds. Secure all valuables, remove jewelry (except wedding bands and religious medallions) in the security tray. Place photo I.D. in I.D pouch, place I.D holder around neck. Lock security drawer and place key chain around neck. Leave personal hygiene items/shower shoes on top of bed. One clean pair of underwear, socks, and t-shirt on bed. Trainees will keep remaining clothing and items not needed for processing in their civilian luggage. Place civilian luggage on top of wall locker. Direct Trainees to shower one Trainee per showerhead. Brief Trainees to wear shower shoes. Ensure Trainees with shaving problems do not shave until they receive the appropriate medical appointment. Have Trainees remove contact lenses and wear glasses, if applicable. Contact lenses are not allowed during Basic Training unless no glasses are available. If Trainees do not have glasses, contact lenses may be used only until their military glasses are issued. Trainees will not go to sleep while wearing contact lenses. Lights out. NOTE: AFTER 2100HRS, ALL ITEMS ON THIS CHECK LIST WILL BE COMPLETED WITHIN 1.5 HOURS AFTER ARRIVAL TO THE DORMITORY. UPON COMPLETION OF THE CHECKLIST OR 1.5 HOURS HAS PASSED, MTIs MUST IMMEDIATELY DEPART. Secure cell phones and complete cell phone roster. Note any damages to phones. 4 Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) ZERO WEEK SQUADRON WITHIN 24 HOURS DFAC: Prior to the first meal, teach chow runner and flight dining facility procedures. Mail: Brief Trainees, immediately upon receipt of mail they will have a minimum of 10 minutes to read their mail. Make sure that they provide the correct mailing address to families/friends. Trainees are permitted to write and mail letters daily upon completion of duties and details. MTIs should provide an opportunity for Trainees to mail letters daily. Inform Trainees to tell families not to send mail after the end of 6WOT, mail will not be picked-up during 7WOT. Naturalization Program: Verify list of Trainees asking to become “United States Citizen” and ask if there is anyone else in the flight that did not sign-up at the PRC. Supervisory personnel will forward the updated listing to the Scheduling office NLT COB Thursday, 0WOT. Teach entry controller procedures (0.5 Hrs.). Secure identification tag to luggage. Complete DD Form 2983 Recruit/Trainee Prohibited Activities and 2AF Form 1, Trainee/Student Rights and Duties ▪ Trainees will read, initial, and sign DD Form 2983 Recruit/Trainee Prohibited Activities Acknowledgment and 2AF Form 1, Trainee/Student Rights and Duties NLT 24 hours after arrival. Conduct make-up training for Trainees arriving late to BMT NLT 24 hours after arrival. ▪ Squadron Unit Program Coordinators (UPCs) will ensure unit file plan is IAW AFRIMS and forms are filed and destroyed 1 year after individual has completed training. Squadron’s maintain hard copies or electronically scan forms by week group/flight. Brief Trainees on Medication Procedures: Trainees may only consume/receive medication that has been prescribed by USAF Medical personnel. Exception: Female Trainees may continue to take birth control pills that were prescribed prior to arrival at BMT; but are still required to attend the Birth Control Clinic. Brief Trainees they are authorized to carry all prescribed medications in original labeled containers. Brief Trainees they are prohibited from purchasing or consuming over the counter (OTC) medication, nutritional supplements (e.g., protein supplements, fat burners, etc.), mineral preparations or vitamins. NOTE: Any of the above items brought from home will be disposed of during the health, morale, and welfare inspection. Trainee Health may issue female Trainees prescribed multivitamins during training. Trainees should preferably take these multivitamins during the evening meal with milk or orange juice. Complete 737 TRG FL 2, Disposition of Individual Patient Medicine for Trainees that bring or receive prescription medication from home. Refer them to Individual Duty Medical Technician (IDMT) within two duty days for evaluation. The supervisor conducting the health, morale, and welfare inspection signs 737 TRG FL 2, Disposition of Individual Patient Medicine. NOTE: Once the form is reviewed by medical authorities file it with flight records and destroy after the flight ships. 5 Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) REID Clinic Brief clinic hours: Monday - Friday: 0630 - 1445 and Saturday 0700 - 1100 for Acute Care only. No Trainees will be seen for FOLLOW-UP APPOINTMENTS on Saturdays. NOTE: Reid Clinic opens at 0915 on Wednesdays due to training. Brief Trainees about personal hygiene/grooming requirements: ▪ Trainees must wash their hands utilizing soap and warm water for a 20 second duration at least five times a day. ▪ Trainees will wash their hands with soap and water prior to each meal. ▪ Trainees will use available hand sanitizers (Trainees must use prior to entering field training dining, walking up/downstairs, dinning and classroom facilities). ▪ Every Trainee must shower twice a day during the BMT summer schedule if they attended Physical Training (PT) that day, and at least once a day during the winter schedule for a minimum of 5 minutes. ▪ Trainees will brush their teeth; Trainees will have at least five minutes twice a day. Reinforce preventative health and good hygiene practices. Do not rush them through this process. Additionally, Trainees will floss their teeth at least once per day sometime after the evening meal and before lights out and have an additional five minutes to do so. ▪ Male Trainees will shave prior to falling out of dormitory for the first scheduled appointment (not PT). ▪ Trainees will never share personal clothing or hygiene items. ▪ Trainees will change undergarments and socks daily. ▪ Trainees will not wear the same uniform for more than two consecutive days. ▪ Trainees will use all sinks, drinking fountains, urinals, toilets and showers in the dormitory. ▪ The MTI/latrine crew will not put restrictions on the use of restrooms. ▪ Trainees will sneeze and cough into their elbow and wash their hands or use hand sanitizer immediately afterward. 6 Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) ZERO WEEK SQUADRON WITHIN 72 HOURS Conduct arrival briefing. Health, morale, and welfare inspection will be conducted within the first 72 hours by each Section Chief or FC along with the flight assigned MTIs. Supervisory personnel will confiscate items considered harmful IAW Health, Morale and Welfare lesson. Initial Phone Calls: Ensure all Trainees call and/or text home within 72 hours or as soon as possible. Trainees are provided a minimum of five minutes and phone calls will only be accomplished from 0800 to 2100 hours. Exceptions are made for Trainees that require a phone call to family members outside the continental United States. ▪ In order to ensure the information is heard and understood, maintain a professional environment during this phone call, minimize background noise and distractions. ▪ Trainees will take/send a photo via text message of their complete and correct BMT mailing address sheet to their family members. Conduct initial height and weight, document the data in e-BMT and identify Trainees with low and high Body Mass Index (BMI). Refer Trainees with 18.5 BMI and below in addition to BMI of 30.0 or more to Athletic Trainer and document in e-BMT. Refer to 737 TRG SOP 36-2905, BMT PT Program. Visitors: Brief Trainees visitors are prohibited throughout their training. Trainees in violation of this policy will be recycled. Brief Trainees on the Wingman/Teammate policy. UCMJ Briefing: Flight MTIs may present the presentation however, a SNCO must be present (preferably the squadron First Sergeant). The SNCO will answer the Trainee’s questions and will sign the UCMJ briefing roster. Trainees sign the roster with their payroll signature. Prior to conducting the first PT session, the flight MTI or squadron PT NCO must teach the BMT PT program and provide a demonstration of exercises. Teach/Apply Drill IAW WAS and BMT Form 107. Begin teaching recruit living area. Complete backpack inventory IAW WAS. Inform Trainees the memory work in Cerego and the grade insignia must be memorized NLT the end of 2WOT. Trainees who have missed blood testing during initial screening are prohibited from performing PT until blood testing results are received. Brief Trainees to wear a T-shirt (PT, Operational Camouflage Pattern (OCP), or white) and undergarments to bed. Trainees may wear their PT shirt, shorts and socks (green or white) to bed however Trainees will not sleep in spandex shorts. If the temperature falls below 68 degrees Fahrenheit during lights out, Trainees may additionally wear their PT pants and jacket. Brief Trainees about authorized items they may carry in their backpack and on their person. Identify Trainees with shaving problems (e.g., razor bumps or severe acne) and provide names to send to Reid clinic. 7 Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) MTI demonstrates proper wear of the eyeglass strap to prevent damage to the eyeglasses and ensure comfortable fit. Eyeglass straps are worn on eyeglasses during all training activities except graduation events. Processing: The following processing appointments must be completed IAW WAS. NOTE: Supervisory Leadership will collect from all flights, the names of Trainees that are unable to attend the following processing appointments. The flight MTI will provide a corrected flight roster and leave contact information with clinic personnel prior to departure. Military Pay All Trainees are required to have a photo ID, pen and be placed in roster number order to receive their EZ Pay Cards. Clipper Cuts Flight MTIs must ensure Trainees take their EZ Pay Cards to pay for haircuts. Brief male Trainees they will initially pay for two haircuts at a discounted rate. If they are recycled and require more haircuts, they will pay for additional haircuts. If placed on medical hold, they will pay for all haircuts received while on medical hold status. Trainees being discharged will pay for all haircuts received while awaiting discharge. Initial Clothing Issue Initial BX Visit MTIs brief Trainees on proper wear of OCP’s. Ensure Trainees have their EZ Pay Card when going to pay for their issued items. Trainees are not required to purchase items not on the mandatory list in Cerego on issued tablet. Bring backpack inventory checklist for any missing items. Brief Trainees on what they are purchasing. Ensure Trainees purchase applicable mandatory items (e.g., shaving cream, glasses straps) BEST Briefing (Corrected Flight Roster with highlighted names of absent Trainees) Immunization/Blood Draw (Bldg 7002) Demand Reduction (Urinalysis) (IN SQUADRON) MTIs may encourage Trainees to drink water up to 1.5 hours prior to appointment but will never restrict Trainee access to the latrine prior to drug testing. Trainees must bring a picture I.D. and a Social Security Number (SSN) card to drug testing. If SSN Card is not available, bring a copy of the Trainees shipping orders or contract with a full SSN. The MTI will bring a corrected flight roster with SSN signed by squadron leadership (CC/DO/SEL/FS/FC) to urinalysis. This signed roster is used ONLY for Trainees with no available source of SSN identification. If a flight simply fails to bring their SSN verification to testing, they will be turned away. Trainee make-ups will be on Tuesday/Wednesday/Thursday from 0800 – 1400 at Bldg 7065/Rm 040. 8 Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) ZERO WEEK SQUADRON ACTIVITIES AND TRAINING REQUIREMENTS General Items Civilian clothing must be laundered and dried prior to storing luggage on top of the wall locker. Name Tag: Accomplished during "Admin Time” (DD Form 2983, eBMT Pin, Rosters, etc) and provided to gaining 1-7WOT squadron. Print a computer-generated roster, titled “Name Tag Roster,” and have each Trainee sign verifying the correct spelling of his/her name. Prior to making any name changes -- verify Trainee's name with SSN card and Records/ID processing prior to making a name change on the roster turned into 802 LRS/LGC. MTI will highlight each Trainees last name to include those with multiple last names (e.g., Trejo Sanchez), hyphens in their last name (e.g., Trejo-Sanchez), and special spellings (e.g., TrejoSanchez) to mitigate any spelling errors. Must be turned into 802 LRS/LGRC, Name Tag Element NLT 1000 on Monday, 1WOT. Follow 802 LRS/LGC Name Tag Roster Turn-in Procedures posted at clothing issue. Mark “USSF” on Name Tag Roster for any Trainees that require USSF nametapes. NOTE: Trainees who fail to make corrections on the roster, which results in a misspelled nametag, must order new nametags from the Military Clothing Sales Store (AAFES) or Dry Cleaning and pay for them at their own expense. e-BMT Pin Number: Accomplished during "Admin Time” (DD Form 2983, eBMT Pin, Rosters, etc) and provided to gaining 1-7WOT squadron. Establish e-BMT Trainee pin number for electronic signatures. Brief Trainees that they need to establish a pin number that they can recall. MTIs will not suggest or assign pin numbers for Trainees. Uniform of the Day (UOD). Beginning with the of 0WOT, the uniform of the day (UOD) is identified on the WAS. Brief flights on Dormitory Maintenance Select cleaning crews and assign a Trainee to be in charge. Brief Trainees in charge of details and student leaders, it is prohibited to limit the use of facilities to keep dormitory clean, e.g. limiting use of sinks and urinals in the latrine. Mark rag storage cans “Clean Rags” or “Dirty Rags” to identify contents. Laundry bags (RH&T/Optional in ATC) containing clean or dirty laundry must be stored in the utility room. If wet items are placed in the laundry bags, they will be kept in plastic bag(s) in a laundry bag. Store empty laundry bags neatly folded on shelves. Utility Room ▪ Mark cleaning agent containers to identify their content with the manufacturer’s label. ▪ Ensure Trainees use cleaning agents and their containers only for their intended purpose. ▪ Do not purchase or use commercial cleaning supplies, e.g., air fresheners, floor wax, etc. ▪ Cleaning crews may use diluted bleach to control mold and sanitize the dormitory and latrines. Use bleach at least once a week. Risk Management (RM): Ensure diluted bleach is not used in combination with any other cleaning products (doing so may produce extremely hazardous fumes). ▪ Use of germicidal/disinfectant cleaning agents will be used in IAW the LP, HAZCOM 9 Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) instructions, and any protocols directed by Public/Trainee Health. 10 Addendum 1, 737 TRGI 36-3, Daily/Weekly Activity Checklists (8 Nov 2023) The maximum stock of supply items authorized for each dormitory utility room is as follows: 1 can of metal polish (or equivalent) 2 cans of scouring powder 12 laundry bags 3 hand brooms 10 sponges 4 mops 4 push brooms 1 container of wax 6 scrub brushes 4 dust pans 6 scouring pads 6 bottles of sunscreen 1 mop bucket and wringer (ea) 4 packages of antibacterial soap 6 packages or 6 rolls of paper towels 4 bottle of glass cleaner 10 pairs of rubber gloves 1 bottle of deodorizer 1 bottle of bathroom cleaner 1 box (large) trash liner 1 bottle (ea) cleaner and disinfectant 1 stack feminine hygiene bags 1 box (small) trash liners 1 container (refill) Hand Sanitizer 2 containers of insect repellant 4 bottles of Peroxide Multi Surf 1 bottle of diluted bleach Rubber bands/twist ties for bundling hangers (1 bag or container for each) Laundry Detergent Pods (Plastic bins are acceptable for storage) Maintain 20 small or 6 large rolls of toilet paper (30 small or 8 large rolls for female dormitories) NOTE: Squadron Commanders may authorize additional items via policy letter (a dated copy will be maintained at 737 TRG/CCV) that may be displayed inside the dormitory utility closet, e.g. hangers, bed aligning tools, plunger, etc. Lessons/Application The following items must be taught, and application conducted IAW WAS and POI Part I & II. The squadron requiring escorts will provide the escorting squadron the number of Trainees that will be needed NLT Saturday 1100 (1 Trainee guide or escort per flight). ALL Airmen/Guardians will report to the PRC at their designated technical training chalk time to include the Do Not Ship (DNS) list and/or in Holdover Airman (HOA) status.
