Study Notes

Upper Respiratory Tract Infections

  • Upper respiratory tract infection (URTI) is an infection of the upper airway components, such as rhinitis (nose), sinusitis (sinuses), common cold (nasopharyngitis), pharyngitis (throat), epiglottitis (upper larynx), laryngitis (larynx), laryngotracheitis (larynx and trachea), and tracheitis (trachea).

Bacterial vs. Viral Infection

  • Initially, URTIs are typically viral, potentially progressing to a secondary bacterial infection.

Viral Infection

  • Symptoms include: itchy or scratchy throat, mild body pain, fever lasting 2-3 days, clear sputum.

Bacterial Infection

  • Symptoms include: blocked throat (difficulty swallowing), fever lasting more than 3 days, colored, thick sputum, severe body pains.

Adult Treatment

  • Augmentin 1g (Amoxicillin+Clavulanate Potassium): One tablet twice daily for 7 days, taken with a full glass of water with or after food. Clavulanic acid enhances Amoxicillin's effectiveness and stability. Allergy to penicillin alternative: Klacid 500 mg (Clarithromycin), one tablet twice daily for 7 days.

Pediatric Treatment

  • Augmentin 457mg Suspension: For otitis media, sinusitis, lower respiratory tract infections, and severe infections, dose is 45mg/kg/day divided into 2 doses. For less severe infections, the dose is 25mg/kg/day divided into 2 doses.

  • Omnicef Suspension (Cefdinir): Dose is 7mg/kg suspension every 12 hours or 14mg/kg once daily for 5 days with meals. Two-hour gap between Omnicef and antacids/iron-containing formulas.

  • Suprax Suspension (Cefixime): Dose is 8mg/kg/day, administered as a single dose or divided into 2 doses.

Probiotic Supplements

  • Probiotics, like AMS Florabiotix 50 Billion, are suggested to be taken two hours before or after antibiotics.
  • It is advised for those who have had antibiotic-related diarrhoea in the past, those taking antibiotics for 5-10+ days, or those switching antibiotics.
  • Probiotics, such as Proflora (Baby Drops and Start), are Microencapsulated; this protects them from stomach acid and ensures they reach the intestines, before releasing.

Cold, Flu, Cough, and Sinusitis

  • Questions to assess the case and recommend medication: Congestion or runny nose, color/type of nasal drainage, fever, headache, difficulty swallowing, cough, frequency of coughing at night, and if the patient is diabetic or a smoker.

Sore Throat

  • Symptoms include pain, itching, dry mouth, hoarseness of the voice, difficulty swallowing, fever, and greenish sputum.

  • Treatment includes antibiotics (for bacterial infections), gargling with Betadine Mouthwash 250ml, lozenges (e.g., Comvita Manuka Honey with propolis), and sprays (e.g., Comvita throat spray). Betadine Throat Spray and Voltfast sachets also used for pain and inflammation. Contraindications for Betadine: Allergic to iodine, under six years, thyroid disorders, and pregnancy/breastfeeding.

Cough

  • Comvita Original Cough Syrup and Comvita Blackcurrent Cough Syrup are options.
  • Dosages for adults and children are provided, with specific instructions for dry or productive coughs.

Sinusitis

  • Symptoms include a runny or stuffy nose, pressure between the eyes and cheeks, nasal discharge (yellow or green), and cough producing mucus.

  • Treatments: Gloclav 1g or Ziquin 500mg (with fever and colored drainage), Clarinase (decongestant), Sterimar Nasal Spray, corticosteroid sprays (Avamys Nasal Spray), Nat Sinus Clearing, and Voltfast.
