Purpose of Guidelines
- The purpose of these guidelines is to provide a framework for Toronto Fire Services personnel to follow when completing forms at emergency incidents.
Responsibility
- All personnel are responsible for being familiar with and following these guidelines.
- The apparatus driver is responsible for ensuring copies of the Immediate Action form and Action Taken form are carried on the apparatus.
Immediate Action Form
- The Immediate Action form is a triplicate form.
- The form can be requisitioned from the Quartermaster Section.
- The three copies of the form are to be distributed as follows:
- Original to the property owner/building representative.
- Canary copy to the Fire Prevention Division office in the Command where the call occurred.
- Pink copy to the District Chief in the District where the call occurred.
- The form is used in situations requiring immediate action by the Fire Prevention Division, such as:
- No Fire Safety Plan.
- Hoarding.
- Fire Alarm or Sprinkler System shut down.
- Blocked Fire Department Access.
- Blocked Exits.
Action Taken Form
- Copies of the Action Taken form can be printed from the TFS Intranet _ Forms Section.
- The completed form is to be provided to the property owner/building representative.
- The form is used when:
- TFS has shut down any building services requiring qualified 'third party' reconnection.
- TFS has caused damage during mitigation of an emergency incident to a building or other property.
