Purpose and Responsibility
- The guideline provides a framework for Toronto Fire Services personnel to initiate repairs or improvements to equipment, acquire supplies, and report lost, damaged, or found equipment.
- All personnel are responsible for bringing repair, requisition, or lost/damaged equipment reports to the attention of the Company Officer/Support Division Captain.
Repair Orders

Repair Orders are used to initiate repairs to equipment.

Equipment to be repaired must be tagged with a written description of the deficiency.

Company Officers/Support Division Captains are responsible for completing and signing Repair Order forms.

Repair Order forms are forwarded to the District Chief for review and signature, then to the Platoon Chief/Division Chief.

A record of all Requisition forms relating to the apparatus shall be
documented in the Apparatus History Report and Record Log or Fire
Station/Support Division Facility Log Book. Entries shall include the following;
(a) Requisition number,
(b) Detailed description of equipment or supplies being ordered,
(c) Name of the individual submitting,
(d) Apparatus,
(e) Date.
Requisitions
- Requisitions are used to order equipment and supplies for apparatus.
- Requisition forms must be legible and provide a clear description of the material being ordered.
- The fire station Duty Officer/Support Division Captain is responsible for completing and signing Requisitions.
- Requisition forms are forwarded to the District Chief, who then forwards them to the Quartermaster Section.
Lost or Damaged Equipment Report
- The report is completed for lost, damaged, or found equipment.
- The Company Officer/Support Division Captain ensures the report is completed and forwards it to the District Chief.
- The report is used to document lost, damaged, or found equipment, including a detailed description of the item, the name of the individual submitting, and the date.
Search and Reporting Process

A thorough search is conducted for lost equipment, including recent incidents, spare apparatus/vehicles, and other apparatus/vehicles that responded to the same call.

The Company Officer/Support Division Captain conducts the search and completes the Lost or Damaged Equipment Report.

The report is reviewed by the District Chief, who ensures a satisfactory search has been conducted.

The Platoon Chief/Division Chief reviews the report, ensures all Company Officers/Support Division Captains have conducted a complete search, and initiates corrective or disciplinary action if required.

The signed report is forwarded to the applicable repair facility.
