### Requesting Survey and Cost Estimate
- All requests for survey and cost estimate require a Service Memo prepared by the District CWDO or IC3.
- The District CWDO must inform the consumer of the following:
- Checklist of requirements based on request
- Approved HLURB for planned subdivisions
- Survey fee of Php 5,000.00 (exclusive of VAT) for the first three poles, payable before the survey
- Additional fee of Php 250.00 (exclusive of VAT) per pole if more than three poles are recommended
- Approved cost estimates are valid only for the applied year unless improvements require a new survey
- A service fee of Php 1,500.00 (exclusive of VAT) is required for an updated cost estimate within the applied year if no new survey is needed
- The ASD forwards the completed Service Memo to the CPD once payment is secured.
- The CPD evaluates the completeness of the requirements, schedules the survey, and updates the Service Memo Database.
- The CPD prepares the staking sheet and cost estimate after the survey, and updates the Service Memo Database.
- The CPD forwards the approved cost estimate to the ASD for implementation.
- The ASD informs the consumer of the progress and collects any additional payment required.
New Connections, Line Extensions, and Pole Replacement/Transfer/Removal
- New connection, line extension, and pole replacement/transfer/removal requests require a Service Memo prepared by the District CWDO or IC3.
- The District CWDO must inform the consumer of the following:
- Checklist of requirements based on request
- Letter of Intent or LGU Resolution
- Signed Road Right of Way Form
- Photocopy of Land Title for the Road Right of Way
- Photocopy of one (1) government-issued ID of the land owner of the Road Right of Way Title
- Photocopy of one (1) government-issued ID of the consumer (account owner of kWh meter or authorized representative for private organization and government agency)
- Duly notarized Special Power of Attorney / Sworn Statement / Secretary’s Certificate / Appointment for authorized representatives
- Approved Electrical Plan
- Approved HLURB for planned subdivisions
- The ASD forwards the completed Service Memo to the CPD.
- The CPD evaluates the completeness of the requirements, schedules the survey, and updates the Service Memo Database.
- The CPD prepares the staking sheet and cost estimate after the survey, and updates the Service Memo Database.
- The CPD forwards the approved cost estimate to the implementing office (ASD/TSD) for implementation.
- The ASD/TSD informs the consumer of the progress and updates the Service Memo Database with payment information.
### Transformer Recommendation Request
- Requests for a transformer recommendation are submitted to the CPD and must include the following:
- Service Memo
- Approved House Wiring Inspection Report
- Pictures of the vicinity
- Pictures of the tapping pole/pole number
- The CPD evaluates the electrical plan and load to determine the transformer requirement based on the following:
- Transformer loading is generally recommended at 80% of its total capacity.
- Primary metering is required for consumers with a total connected load of 190 amperes and above (computed at 240v secondary - three or single-phase load).
- Secondary metering (three- or single-phase load) is used for consumers with a total connected load above 90 Amps and below 190 Amps.
- The CPD forwards the approved Transformer Recommendation to the TSD for implementation.
- The TSD informs the consumer of the progress.
- The ASD updates the Service Memo Database with payment information.
### Multimeter Installation
- If an establishment owner refuses to install a mother meter under their name, the occupant applying for the electric connection needs to execute a tripartite agreement. CPD recommends a mother meter if no agreement is submitted by the establishment owner.
