Definition of Shock
- Shock is a condition resulting from a depressed state of many vital body functions due to decreased tissue perfusion.
- It is a life-threatening condition that can be reversible or irreversible.
- Shock can be classified into different types, including Cardiogenic shock, Distributive shock, Traumatic shock, Electrical shock, Insulin shock, and Hypovolemic shock.
Classification of Shock
- Hypovolemic shock: results from loss of circulating volume from hemorrhage, common in trauma/surgical patients, and can be Hemorrhagic or non-hemorrhagic.
Cardiogenic Shock
- Cardiogenic shock is a circulatory pump failure leading to diminished forward flow and subsequent tissue hypoxia.
- Mortality rate is up to 50-80%.
- Common cause is Myocardial Infarction (MI).
Vasodilatory Shock (Septic Shock)
- Vasodilatory shock is hypotension resulting from failure of the vascular smooth muscle to constrict appropriately.
- Most frequently encountered form of vasodilatory shock is septic shock.
Septic Shock
- Septic shock is a result of INFECTION, BACTEREMIA (presence of viable bacteria in the blood), and SIRS (Systemic Inflammatory Response Syndrome).
- SIRS is the systemic inflammatory response to a wide variety of severe clinical manifestations, characterized by:
- Temperature > 38°C or less than 36°C.
- Pulse rate > 90bpm.
- Sepsis-induced hypotension is characterized by systolic blood pressure < 90mmHg.
