Disciplinary Action Recommendation (BSO A#18)
- Used when discipline rather than counseling is recommended
- Employee must acknowledge receiving a copy by signing the original form
- Employee's signature only confirms receipt and discussion of the recommendation
- Failure to sign after being directed may lead to further disciplinary action
- If employee refuses to sign, supervisor will write "Refused" and date/time of refusal
- Witness may sign and date the report below the employee's signature area
- Reports will not be copied for inclusion in other files without Internal Affairs' approval
- Internal Affairs is responsible for report distribution and compliance with Florida Statutes on archiving disciplinary reports

- Records information relating to employee Disciplinary Action Recommendations
- Directors or designees are responsible for maintaining the log
