Personnel Accountability Report (PAR) is a positive acknowledgement to a supervising authority that a person or unit is present or in attendance.
Site Credentials are official accountability tags, IDs, cards, or licenses that prove an individual is qualified to be on site and/or perform certain functions.
Authorized Site Visitors are qualified emergency responders that are neither from the department nor mutual aid, and have a valid business reason to be present at the incident.
Freelancers are emergency responders operating independent of supervision.

Levels of Accountability

The PAS level activated depends on the size and magnitude of the incident, and the type and degree of hazards members are exposed to.
There are three levels of personnel accountability: Unit Level, Command Level, and Enhanced Command Level.
Unit Level Accountability is implemented during the initial stages of incident operations, and Unit/Crew Leaders maintain crew integrity and situational awareness.

Unit Level Accountability

The first arriving unit serves as the initial collection point for all Unit Riding Lists (URLs).
The identity and assignment of crew members are immediately reported to the IC through the appropriate supervisory levels.
When notified of an unaccounted member or critical life safety condition, the IC initiates appropriate emergency actions.

Demobilization

No individual is to collect or manipulate any accountability device without authorization from the IC or designee.
No unit or crew is to leave without being properly released by command.
Units retrieve Unit Riding Lists (URLs) from the IC or designee upon demobilization.

Task Level: Requires knowledge of the location of each firefighter and what they are doing at all times, where unit/crew leaders must maintain a current Unit Riding List (URL) of all personnel responding on the unit.

Quality and Accountability

All personnel are accountable for adhering to the SOP.
Governance section outlines quality control measures to ensure the implementation and growth of the SOP.

Program Evaluation

Effectiveness of the Respiratory Protection Plan will be evaluated, and corrective actions taken to ensure the plan is properly implemented.
Hanover Fire-EMS will regularly consult with personnel to assess their views on the effectiveness of the plan and identify areas of improvement.

Accident/Incident Reporting Program

The department head or designee shall lead the incident review for all OSHA recordable injuries, significant near misses, and/or major property damage events occurring within their department.
A Root Cause Analysis Team Leader will be assigned to investigate the incident, determine root causes, and establish corrective actions to prevent reoccurrence of similar events in the future.

Personnel Accountability System

Upon arrival at an incident scene, the officer/crew leader must place the Unit Riding List (URL) in the first arriving unit's officer's seat.
If a unit/crew is assigned to a remote area, the URL shall remain accessible and visible until collected by the accountability assistant.

Command Level Accountability

The Incident Commander's (IC) designee retrieves the Unit Riding Lists (URLs) from the officer's seat and takes them to the command post.
