Motivation and Adherence

  • Motivation is internal and external factors that drive a person toward action.
  • Adherence is sticking to or being faithful to something, like a diet or school rules. Nonadherence is failing to follow planned recommendations.
  • Motivation can be intrinsic (coming from within) or extrinsic (coming from outside). Examples of intrinsic motivation include acceptance, curiosity, independence, power, social contact, and social status. Examples of extrinsic motivation include recognition, benefits (like perks and bonuses), and punishment. Another extrinsic motivation is incentives. This form of motivation involves rewards like bonuses and promotions. Fear is also considered a form of motivation where punishment or negative consequences are used. Achievement motivation involves the drive for competency, and external recognition. Growth is the need for self-improvement and increasing knowledge of oneself and the outside world. Power involves the desire to control or affect the behavior of others. Social motivation includes the desire to belong to a group and be accepted.
  • Factors influencing behavior change include predisposing factors (knowledge, beliefs, attitudes based on life experiences, gender, age, race, socioeconomic background), enabling factors (skills, abilities, and resources available), and reinforcing factors (support, encouragement, or discouragement from others).
  • Motivational techniques include positive imagery (posting motivational themes), team-building activities (encouraging teamwork outside the office), training (teaching how to deconstruct tasks), enhanced communication (talking and listening to team members), and targets, rewards and incentives (financial rewards or increased responsibilities).
  • Assessment of motivation includes cognitive variables (capacity to learn, readiness to learn), affective variables (expressions of constructive emotional state, moderate level of anxiety), physiological variables (capacity to perform required behavior), experiential variables (previous successful experiences), and environmental variables (appropriateness of physical environment, social support systems).
  • Steps to motivate students include relaxation, activating students, encouraging attentiveness, facilitating fun, altering the classroom atmosphere, creating learning situations, controlling noise levels, improving concentration, inspiring right-brain creative thinking, strengthening critical thinking, and enhancing verbal and emotional intelligence.
  • The 5 As of adherence in counseling include Ask (understanding patient knowledge), Advise (correcting misunderstandings), Agree (deciding on a course of action), Assist (co-developing a plan), and Arrange (scheduling follow-ups). These 5 As are used in blood pressure counseling.
