- Common symptoms to assess: chest pain (CP), shortness of breath (SOB), headache (HA), fever, chills, cough, nausea/vomiting/diarrhea (N/V/D), abdominal pain, dizziness, or lightheadedness.
Chest Pain Pertinent Negatives
- Evaluate for: SOB, HA, fever, chills, cough, N/V/D, diaphoresis, jaw, back, and lower extremity pain/swelling.
- Assess for recent travel, surgery, or prolonged immobilization.
- Confirm absence of ripping or tearing sensations in the chest, back, abdomen, or pelvis.
SOB Pertinent Negatives
- Assess for CP, HA, fever, chills, cough, and back pain.
- Evaluate for N/V/D, abdominal pain, lower extremity pain/swelling.
- Consider recent travel, surgery, immobilization, hematochezia, melena, dizziness, or lightheadedness.

Fall Pertinent Negatives

- Confirm that the patient did not hit their head.

Syncope/Seizure Pertinent Negatives
- Assess for prodromal symptoms: chest pain, SOB, dizziness, lightheadedness, or palpitations.
