    Personal Accountability Tags (PAT)

    • Firefighters are issued 4 yellow tags with black print

    Level I Accountability (Single Company Incidents)

    • Passports stay on the apparatus, driver takes accountability.

    Level II Accountability (Multiple Company Responses)

    • Each company leaves one passport on the vehicle
    • Second passport goes on the Status Board at the designated location.
    • First vehicle on each side anchors the initial accountability location for following companies
    • Division/Group Supervisors manage accountability as the incident escalates
    • If Division/Group Supervisors are in the hazard zone, passports stay outside, with a designated officer.

    Level III Accountability (High Rise Incidents)

    • All companies bring both passports and small status board to the lobby
    • Two accountability locations: Lobby Control and Forward
    • Lobby Control: First company puts their small status board in the lobby, Incident Commander designates accountability person
    • All companies entering the lobby put a passport on the lobby board
    • Chief Officers bring large status boards for escalating incidents.
    • Forward Accountability: Located below the hazard zone, records personnel in the zone. Hazard zone is the fire floor and above. Second Engine of the Fire Attack Group brings its status board.
    • Companies advancing past the floor below the fire leave their passport at Forward Accountability.

    Personnel Accountability Report

    • Report of personnel assigned to the hazard zone. . Confirming members are accounted for (crew leader). . Keeping track of all company members.
    • Incident Commander account for all hazard zone personnel
    • On small incidents, crew leaders report to IC
    • On multiple incidents, crew leaders report to Division/Group Supervisor
    • Crew leaders account for all personnel whose tags are on the passport.

    Missing Firefighter

    • An absent member is assumed lost or trapped
    • Crew leader reports missing member to Incident Commander immediately
    • Incident Commander initiates: requesting more alarms, roll call, Rapid Intervention Team (RIT) search, adjusts strategies to a priority search and rescue, reposition operating companies.

    Terminating Passport Accountability

    • Passport accountability continues until "Situation Contained" is reported
    • If contained, Incident Commander designates when to terminate.
    • On release, crew leaders ensure passports are returned.

    Two-In/Two-Out Rule

    • First arriving crews operate with at least 4 firefighters
    • Minimum of 2 inside the hazard zone. At least 2 outside for assistance or rescue.
    • Standby members constantly monitor hazard zone members
    • One standby stays in radio contact.
    • Standby can do other roles (apparatus operator, IC)
    • Standby rescues operating crews as needed
    • Incident Commander announces use of standby members on B# FG talkgroup.
    • If an incident is not stabilized, a Rapid Intervention Team is formed.

    Rapid Intervention Team

    • RIT Group established for all incidents beyond initial stages, especially with interior attacks.
    • Group maintained until "Situation Contained".
    • Incident Commander replaces RIT members if required.
    • RIT Group is made of personnel not doing other firefighting duties and is ready for immediate deployment.
    • Location is at Incident Commander discretion, usually close to Command Post.
    • RIT can stretch and charge an attack line.
    • Incident Commander may designate RIT Supervisor, otherwise, senior officer of the group does it.
    • RIT location and assignments announced on the appropriate talkgroup.

    Equipment

    • Minimum equipment varies by structure type (e.g., ropes, saws, ladders, thermal imagers, hand tools).

    Emergency Evacuation

    • Signal transmitted when all personnel must leave the hazard zone due to safety issues
    • Personnel do not reposition lines or retrieve equipment during an evacuation
    • Reasons for urgent messages: fire condition changes, collapse potential, backdraft, explosion, or other hazards determined by Incident Commander

    Air Horn Blast

    • Air horns of all apparatus near hazard zone blare 10 seconds on, 5 seconds off, 10 seconds on
    • This signals an evacuation

    Rehab Group

    • Rehab Group is activated under specific conditions (IC request, Safety Officer request, second alarm, working hazmat)
    • Group is supervised by the 2nd EMS Supervisor or someone directed by IC.
    • At least one dedicated transport vehicle with two medics assigned
    • The Rehab Group reports to the Safety Officer or Incident Commander (no Safety Officer)
    • Location: clear from incident stress, warm/dry in cold/wet weather, cool/moist in hot weather. High rise fires: floor below fire (per High Rise SOPs)
    • Functions: Hydration (water first, then other drinks), Nourishment (consider foods after 4 hours, or mealtime), Medical evaluation (BP, pulse, oxygen, and other symptoms).
    • Passports turned in to the Rehab staff when entering. To be retrieved when exiting.

    Staging Levels

    • Level I Staging applies to all multiple company responses on the first alarm
    • Level II Staging applies to greater alarm responses and large incidents needing an on-scene reserve.

    Fire Alarm Assignments

    • Fire Alarm "A": single-family and small commercial structures—2 companies (engine and ladder)
    • Fire Alarm "B": apartment buildings, hospitals, large commercial structures—3 companies (engine, ladder, Battalion Chief)
    • Fire Alarm "High-Rise": high-rises—multiple companies, per high-rise protocol (Level III accountability)

    Staging Area

    • Formal staging area established when the Incident Commander deems necessary, usually for greater alarm responses and larger incidents.
    • Companies stage until assigned by the Staging Area Manager
    • Initial Staging area location is the first company, who then guides others.
    • The Staging Area Manager relays company assignments to divisions or groups. The manager also relays the area, talkgroup and supervisor assignments.
    • The Staging Area Manager maintains accountability of resources and informs IC when resources are low (2 engines, 1 ladder or less). May have to guide response to the staging area.

    Incident Command

    • First arriving company or division member establishes command
    • Initial Incident Commander develops and implements incident action plan (investigation or fire attack).
    • Command Post generally established at the front or address side of the incident.
    • Location is to be announced over the radio.

    Fire Attack Group

    • First two engines and first ladder form the Fire Attack Group
    • Incident Commander designates a supervisor for this group
    • Finding the fire floor, designating attack stairwell and evacuation stairwell are key functions
    • Fire Attack Group objectives: put an attack line in service, rescue people near the fire, assess the fire area

    Lobby Control Group

    • Takes control of Lobby Accountability
    • Maintains and activity log, and records company designations and assignments for those entering the building.
    • Gets elevator and stairway keys.
    • Evaluates elevator operations, fire protection systems (fire pump discharge pressure), and other systems (HVAC, electrical)

    Upper Search and Evacuation Group

    • Second ladder and rescue create the USE Group, responding based on incident needs.
    • Primary search areas: attack stairwell, evacuation stairwell, floor above fire, top floor, elevators, other suitable areas.
    • Communicating with Fire Attack Group needed for tactical plan and search areas.
    • Controlling stairway doors and other openings. Reports to Incident Commander

    Resource Group

    • Establishes a forward staging location, located at least 2 floors below the fire.
    • Maintains an activity log, reports to IC or Operations Chief
    • Manages companies and equipment present
    • Resource Area Supervisor needs to take account of equipment needs

    Stairwell Support Group

    • Assigned companies to move equipment from the lobby to Resource area, for 2 floor segments or more. . Limits PPE to essentials in this area.

    Ventilation Group

    • Established for buildings needing more ventilation (multiple resources)
    • First priority on ventilation is shutting down and/or providing operational control of the HVAC systems.
    • Coordinated ventilation is vital. Avoid breaking windows on higher floors.
    • Ventilation coordinated with Fire Attack.

    Medical Group

    • Established if medical needs exceed the Incident Commander's control.
    • Responsible for triage, treatment, and transport of victims to the Casualty Collection Point (CCP)
    • CCP is at least below two floors below fire zone, near the Resource Group
    • Coordinating ground level treatment and transport

    Water/Flood Incidents

    • Boat/water rescue crews (2 max in moving water, as appropriate in static water).
    • Rescue technicians wear specific personal flotation devices. Additional safety gear may be required.
    • Reach, Throw, Row, Go is the sequential order for water rescues.
    • Static water situations can include ponds, lakes, floods, and reservoirs
    • Moving water is greater than 1 knot speed.
    • Prioritizes evacuation and shelter-in-place for floods.
    • DART/Dive crews coordinate with the IC
    • Taglines are used, and only the appropriate carabiners.
    • Ice rescue specific PPE requires taglines.
    • Floodwater rescues must account for drain structures and Delta-P hazards.

    Hazardous Materials Incidents

    • Separate hazard zone for suspected explosive device or package.
    • First arriving unit establishes command, sizes up situation, gives radio report, and allocates resources
    • Apparatus position considered distance from the fire, upwind/uphill when possible
    • Pump operator manages water supply. Personnel in Hazard zone must be in full PPE, including SCBA.

    Carbon Monoxide Incidents

    • First reading outside the building, second inside.
    • Monitor levels in all suspicious areas.
    • If readings above safe levels (35 ppm or greater): evacuate, use SCBA
    • Investigate the source. Turn appliances/furnace on.
    • Notify Gas Company for gas related problems
    • Document all actions on form FP-356

    Vehicle Accidents

    • Incidents classified as Vehicle Accident with Injury, possible entrapment/high risk
    • Extrication companies are trained to use special tools, with the most senior/ranking person directing extrication
    • If necessary, upgrading to high risk incident in case of simultaneous use of multiple hydraulic rescue tools, stabilization equipment other than step chocks

    Elevator Safety

    • Elevators are not used in some incidents (fire on 5th floor or below, no PPE/SCBA/radio/keys).
    • Ensure elevator is in fire service mode. Avoid service elevators in initial phase.

    Small Spills

    • Confinement, absorbents (avoid flushing water).
    • Debris from vehicle accidents are dealt with appropriately by appropriate personnel

    Large Spills

    • Notify EPA and County EMA when spills exceed 25 gallons or enter waterways
    • Clean up is under the direction of authorities.
