Study Notes
Perioperative Nursing Overview
- Encompasses the entire surgical experience using the nursing process.
- Divided into three phases: preoperative, intraoperative, and postoperative.
Perioperative Phases
- Preoperative Phase: Begins with the decision for surgery and ends with the transfer to the operating room.
- Intraoperative Phase: Starts upon transfer to the operating room and concludes with admission to the Post Anesthesia Care Unit (PACU). Roles may include scrub nurse, circulating nurse, or registered nurse first assistant.
- Postoperative Phase: Begins with admission to PACU and lasts until follow-up evaluation in a clinical setting or home.
Operating Room Zones
- Unrestricted Areas: Allowed to wear street clothes; includes changing rooms.
- Semi-restricted Areas: Scrubs and slippers required; includes hallways and staff break rooms.
- Restricted Areas: Strict sterile protocols; includes operating room and sterile storage. Masks, caps, and goggles required.
Preoperative Care
- Informed Consent: Essential for autonomy; the surgeon must explain the procedure and answer questions.
- Monitoring Postoperative Recovery:
- Assess incision sites for dehiscence, and ensure drains are patent with minimal output before removal.
- Gastrointestinal Monitoring: Track intake/output, manage nausea, and ensure NPO status until gag reflex returns.
- Renal Monitoring: Urinary output should exceed 30 mL/hr, with expectations for urination within 6-8 hours post-surgery.
Extended Postoperative Care
- Continue assessment and observation for infection: watch for redness, swelling, fever, or elevated white blood cells.
- Encourage mobility and active range of motion exercises every two hours to support recovery and peristalsis.
Patient Education and Support
- Splinting the Incision: Instruct patients to support their incision during deep breathing and coughing.
- Stress the importance of not tugging on invasive devices like Foley catheters or NG tubes.
- Psychological Preparation: Recognize patient anxiety and offer reassurance, answering questions and respecting privacy.
Preoperative Checklist
- Confirm patient identification with a bracelet.
- Assess for allergies, including latex allergies.
- Verify that all preoperative checklist items are completed.
- Ensure informed consent is signed for the operation and any additional procedures.
