Parenteral Administration: Intradermal, Subcutaneous, and Intramuscular
- Intradermal route: Preferred sites include upper chest, inner forearms, and back.
- Two methods used for allergy testing: intradermal injection and skin prick test (SPT).
- Subcutaneous route: Medication deposited in loose connective tissue. Common drugs include heparin, insulin, and enoxaparin. Common sites: upper arm, anterior thighs, and abdomen.
- Intramuscular route: Injection deep into muscle mass. Common sites: vastus lateralis, rectus femoris, ventrogluteal, dorsogluteal, and deltoid.
- Age and muscle mass are factors for IM injections.
- Volume of medication: for adults 0.5 to 2 mL, pediatric patients 1 mL or less.
- Needle length for adults: 1 to 1 ½ inches. Gauge: 20-22
- Important sites for IM administration in infants: Vastus lateralis. In adults: Gluteal area, deltoid, vastus lateralis.
- Deltoid muscle: Maximum volume 1 mL; landmarks: acromion process and axilla.
- Z-track method: Suitable for irritating or staining medications.
- Documentation: Record date, time, drug name, dosage, site, route and patient's response to drug therapy. Regular patient assessments to note therapeutic effectiveness. Document and report adverse drug effects. Validated patient education.
