Study Notes

UWORLD REVIEW-TB and Airborne Precautions

  • Use a surgical mask, not an N95, for clients
  • This protects healthcare workers and clients from respiratory secretions
  • Healthcare workers transporting clients from negative pressure rooms to other locations need an N95 mask to protect themselves

Cirrhosis of the Liver

  • Lab results will show elevated ammonia, bilirubin, and prothrombin time
  • Elevated bilirubin (jaundice) is due to the liver's decreased ability to conjugate and excrete bilirubin
  • Most coagulation factors are produced in the liver. A cirrhotic liver cannot produce these factors, resulting in elevated PT, aPTT, and INR levels

Total Knee Replacement

  • Recent/current infection is a contraindication to total knee replacement surgery
  • Wound infection is more likely in clients with pre-existing infections
  • Investigate burning on urination as it may indicate a UTI
  • Stop NSAIDs (including selective COX-2 inhibitors) 7 days prior to surgery to decrease the risk of intra/post-op bleeding

Transfusion Reaction

  • Stop transfusion immediately
  • Maintain an IV line with normal saline
  • Monitor vital signs
  • Notify healthcare provider and blood bank
  • Recheck client's blood type and tags
  • Return bag to blood bank for further testing
  • Collect blood and urine samples to evaluate hemolysis
  • Complete facility paperwork

Chronic Kidney Disease with Large Pleural Effusion

  • Decreased fremitus
  • Diminished lung sounds
  • A pleural effusion is an abnormal collection of fluid (>15-20 ml) in the pleural space, preventing full lung expansion
  • Results in decreased lung volume, atelectasis, and ineffective gas exchange
  • Other symptoms: dyspnea, nonproductive cough, diminished breath sounds, dullness to percussion, decreased tactile fremitus, and trachea deviation to the opposite side in large effusions
  • Sounds travel faster in solids (consolidation) than in an aerated lung, resulting in increased fremitus in pneumonia
  • Fluid or air outside of the lung interrupts sound transmission, leading to decreased fremitus in pleural effusion and pneumothorax

Cancer

  • Unintentional weight loss of >10% of usual weight (in nonobese clients) requires evaluation and could indicate underlying cancer. Anorexia, and altered taste sensations are also clinical features of cancer and contribute to weight loss.
  • Clients with cancer should be evaluated using the CAUTION acronym: Change in bowel or bladder habits A sore throat that does not heal Unusual bleeding or discharge from body orifice Undetectable thickening or lump Undetectable change in a wart or mole Undetectable nagging cough or hoarseness Change in appearance of a mole or breast tissue
