Mesencephalon (Midbrain)
- The mesencephalon is the shortest part of the brainstem.
- It's approximately 2 cm in length.
- It is located between the pons and the diencephalon.
Relations
- Inferior: Pons
- Posterior: Cerebellum
- Superior: Diencephalon
External Features - Ventral Surface
- Crus cerebri: Cerebral peduncles.
- Interpeduncular fossa: Located between the crus cerebri.
- Oculomotor nerve (III): Exits from the interpeduncular fossa.
- Posterior perforated substance: On the surface of the interpeduncular fossa, behind the crus cerebri.
External Features - Dorsal Surface
- Superior colliculus: Involved in auditory pathways.
- Inferior colliculus: Involved in visual pathways.
- Brachium of superior colliculus: Connects the superior colliculus to other structures.
- Brachium of inferior colliculus: Connects inferior colliculus to other structures.
- Cranial nerves: Trigeminal (V), middle cerebellar peduncle, vestibularcochlear (VIII), facial (VII), inferior cerebellar peduncle, hypoglossal (XII), accessory (XI), and hypoglossal (XII) present in the region
Internal Structure of Mesencephalon
- Divided into three parts: crus cerebri, tegmentum, and tectum (lamina tecti).
- Crus cerebri: 2 white matter structures formed by axons emerging from cerebral hemispheres. Contains frontopontine, corticospinal, occipitopontine, parietopontine and temporopontine fibers.
- Tegmentum: Located between the substantia nigra and the tectum. Contains the cerebral aqueduct, periaqueductal gray substance, red nucleus.
- Tectum (lamina tecti): Posterior to the cerebral aqueduct. Contains the superior colliculus (auditory) and inferior colliculus (visual).
Cranial Nerves at Mesencephalon Level
- Oculomotor (III): Contains motor and parasympathetic fibers.
- Trochlear (IV): Contains motor fibers.
Inferior Colliculus
- Related to auditory pathway.
- Fibers travel through the lateral lemniscus.
- Brachium of inferior colliculus → medial geniculate body → auditory cortex
- Functions including voice localization and frequency evaluation.
Substantia Nigra
- Located between crus cerebri and tegmentum.
- Dark pigmentation (melanin)
- Composed of Pars compacta (dorsal) and Pars reticularis (ventral).
- Involved in basal ganglia.
Red Nucleus (Nucleus Ruber)
- Part of the reticular formation.
- Dorsomedial to substantia nigra.
- Observed from caudal levels of superior colliculus to caudal levels of diencephalon.
- Parts include pars parvocellularis (rostral) and pars magnocellularis (caudal).
Pretectal Area/Nucleus
- At the level of the posterior commissure.
- Contains the pretectal nuclei which are related to visual pathways.
Cranial Nerve Nuclei
- Parasympathetic nucleus, oculomotor (III), Edinger-Westphal
- Motor nucleus of the oculomotor (III) nerve
- Motor nucleus of fourth cranial (trochlear) nerve,
- Mesencephalic nucleus of the fifth cranial (trigeminal) nerve, GSA
Inferior Colliculus Level
- Decussation of the superior cerebellar peduncle.
- Trochlear nucleus.
