Equipment Repair and Replacement Procedures
- MDFR repairs broken equipment when cost-effective and feasible.
- Each repair/replacement is tagged with crucial details:
- Problem description or reason for return.
- Originating Station or Unit.
- Name and shift of the Officer-in-Charge (OIC).
- Date of service request.
- Repair/replacement tags can be ordered online from the Inventory and Supply Bureau.
Lost & Damaged Equipment Reporting
- An online Lost & Damaged (L&D) form must be completed for equipment needing repair/replacement unless exceptions are made.
- The Unit OIC must document missing, damaged, or new/replaced equipment with asset numbers in the apparatus log book as per Policy and Procedure II-J-3.
Equipment-Specific Repair Procedures

Vehicles and Small Equipment:
- Submit requests via MEB Online Reporting System.
- For out-of-service issues, call Support Office at (786) 336-3100 for specific instructions.

- Submit an Online Service Request.

Mobile Communications Equipment:
- Submit Online Service Requests and follow up with Support Office at (786) 336-3100 if needing further guidance.

- Operations personnel contact their battalion chief for spares.
