Histopathology of Dental Caries - ICDAS Codes

    • ICDAS Codes: International caries detection & assessment score

Moderate Stage Caries → Code 4

    • Dark shadow from dentine with/without enamel breakdown
    • White or brown spot when wet
    • Darkened areas of intrinsic shadow (grey, blue, brown)

Extensive Stage Caries → Code 5

    • A distinct cavity in opaque or discoloured enamel with visible dentine
    • Visible evidence of dentine demineralisation
    • White or brown wells

Extensive Stage Caries → Code 6

    • An extensive cavity
    • Visually able to see extension into dentine

    Recap - Structure of Dentine

    • The mineral component of dentine is hydroxyapatite
    • Organic material is collagen
    • This is a vital tissue as the dentin tubules are permeated with the cell process of the odontoblasts
    • It therefore is able to defend itself from assault (alongside the pulp)
    • We refer to this as the dentinal pulp complex as the 2 tissues are intimately related.
