Completing Incident or Unit Reports
- Fire RMS Incident or Unit Reports must be completed using TFS computers only.
- Company Officers are responsible for completing incident reports as soon as possible after the incident.
- Officers must use their Officer Notebook to record incident information only.
- Handwritten reports are not accepted, and all diagrams must accurately reflect the circumstances of the incident.
Procedures for Completing Reports
- Supplemental hard copy reports, such as Incident Observation Forms (IOFs), must be completed using a station computer.
- IOFs should be completed as soon as possible after the incident.
- The District Chief collects reports, ensuring accuracy before delivering them to the Platoon Chief for final review and sign-off.
- The Platoon Chief forwards IOFs to TFS Investigations or TFS Reports accordingly.
Fire Scene Documentation
- All fire scene documentation, including IC's worksheets, FIT's Running Log, Scribe notes, and diagrams, must be collected by the DC or designate at the conclusion of the incident.
- Collected documentation is forwarded to TFS Reports.
Information Requirements for Reports
- Much of the required information is provided by the TFS Communications Centre on the fire station printer or directly into the report via the CAD/Fire RMS integration.
- Information not automatically populated must be copied exactly as it appears from the dispatch information provided by the Communications Centre.
- Exceptions apply if the incident address differs from the dispatched address; the correct address must be added, along with apartment/unit number if applicable.
Review of Reports
- District Chiefs review all reports, including Company Officer's Fire RMS Incident or Unit Reports and submitted Fire RMS Incident or Unit Reports for incidents not attended by a District Chief.
- The reviewing District Chief is determined by the sequence of events, including who arrived on scene and assumed Command.
- The review process ensures accuracy and sufficient detail in the Company Officer's Narrative, which must reflect what took place and meet evidentiary standards.
Responsibilities and Accountability
- Company Officers are responsible for all incidents they are dispatched to, whether they populate automatically in Fire RMS or not.
- District Chiefs must review reports and follow up with Company Officers to ensure satisfactory reporting.
- The Platoon Chief ensures all Chief Officers complete narratives for significant events and reviews incident reports.
- The Platoon Chief is also responsible for accountability and follow-up with all Officers not in compliance with the guidelines.
