Extramedullary Cyst-Like Conditions of the Spine
- Extramedullary spinal conditions are fluid-filled collections found outside the spinal cord.
- Can be clinically silent or cause compression of the spinal cord or nerve roots.
- These conditions may be referred to as "spinal cysts" but most don't meet the definition of a true cyst.
- They can be categorized into meningeal and non-meningeal lesions.
Meningeal Cyst-Like Conditions
- The meninges are the layers of tissue that surround the brain and spinal cord.
- The space between the pia mater and arachnoid is called the subarachnoid space, containing cerebrospinal fluid.
- Dorsal and ventral nerve roots travel through the subarachnoid space covered by pia mater before piercing the dura mater, taking a sleeve of arachnoid with them.
Intradural Arachnoid Diverticulum
- A rare cause of spinal cord compression in dogs, very uncommon in cats.
- These structures communicate freely with the anatomic subarachnoid space.
- They do not correspond to the more chronic, degenerative entity seen in people.
Extradural Spinal Perineurial (Tarlov) Cysts
- Located within the dural sheath of the spinal nerves.
- Develop within the perineurium, the sheath surrounding nerve fascicles.
Non-Meningeal Cyst-Like Conditions
- More commonly originate from articular process joints.
Articular Process Joint Cysts
- Can be synovial or ganglion cysts.
- Synovial cysts are filled with synovial fluid and communicate with the joint cavity.
- Ganglion cysts are thought to develop due to trauma to the joint and have a more gelatinous fluid.
Ligamentous/Discal Cysts
- Controversial in dogs and cats.
- Discal cysts are ventral intraspinal fluid-filled structures with a wall of dense fibrous connective tissue, communicating with the intervertebral disc.
- They appear as ventral extradural lesions in the vertebral canal immediately adjacent to the intervertebral disc.
- In people, they manifest as chronic painful lumbar radiculopathy.
- In dogs, they have a more acute presentation, are more common in the cervical region, and are typically non-painful.
- The term 'discal cyst' might be inappropriate because these lesions differ in clinical presentation and MRI findings when comparing dogs and people.
