Esophageal Squamous Cell Carcinoma
- Most esophageal squamous cell carcinomas are caused by smoking and drinking alcohol, which work together to increase the risk.
- Esophageal squamous cell carcinoma is also common in areas where alcohol and tobacco use is uncommon.
- Nutritional deficiencies and mutagenic compounds found in fungus-contaminated foods may increase the risk.
- HPV infection may contribute to esophageal squamous cell carcinoma in high-risk regions.
- The molecular pathogenesis of esophageal squamous cell carcinoma is not fully understood, but recurrent abnormalities such as amplification of SOX2 (a gene involved in stem cell self-renewal), overexpression of cyclin D1 (a cell cycle regulator), and loss-of-function mutations in TP53, CDH1, and NOTCH1 are implicated.
Morphology
- Unlike adenocarcinoma, squamous cell carcinomas occur mostly in the middle third of the esophagus.
- Early lesions appear as small, gray-white, plaque-like thickenings that may enlarge into polypoid or exophytic masses.
- They can obstruct the esophageal lumen, thickening and stiffening the esophageal wall.
- They can invade surrounding structures including the respiratory tree, aorta, mediastinum and pericardium.
Histologic Variants
- Most squamous cell carcinomas are moderately to well differentiated.
- Less common variants include verrucous squamous cell carcinoma, spindle cell carcinoma, and basaloid squamous cell carcinoma.
- Regardless of the histology, esophageal squamous cell carcinomas are generally large and invasive at diagnosis.
Lymph Node Metastasis
- Sites of lymph node metastasis vary with tumor location:
- Upper third: favors cervical lymph nodes
- Middle third: favors mediastinal, paratracheal, and tracheobronchial nodes
- Lower third: spreads to gastric and celiac nodes
