Classification

Drug Interactions
- CNS depressants and Tricyclic Antidepressants (TCAs) can cause additive sedative effects.
- Co-administration with phenothiazines and anticholinergic agents may enhance adverse anticholinergic effects.
- There is a risk of a fatal hypertensive crisis when taken with Monoamine Oxidase Inhibitors (MAOIs).
Indications
- Primarily used for short-term management of acute musculoskeletal conditions.
- Off-label use includes treatment for fibromyalgia.
Mechanism of Action
- Acts at the brainstem and spinal cord; believed to block the reuptake of norepinephrine.
- Results in increased norepinephrine activity, producing anticholinergic effects.
- No direct action on skeletal muscle; operates centrally.
Side Effects
- Common: Drowsiness, dizziness, xerostomia (dry mouth)
- Cardiovascular: Orthostatic hypotension, tachycardia, syncope, palpitations
- Gastrointestinal: Nausea, vomiting, diarrhea, anorexia, flatulence

Contraindications
- Hypersensitivity to tricyclic antidepressants.
- Prolonged QT interval conditions.
- Recovery phase after a myocardial infarction (MI).

- Establish a baseline neurologic status and assess pain levels.
- Monitor for fall risk due to drowsiness.
- Observe for signs of urticaria, rash, or pruritus.
