- Exchanging information or feelings between two or more people, a basic component of human relationships, including nursing.
- Essential for establishing a nurse-client relationship, collecting assessment data, initiating interventions, evaluating outcomes, and promoting health.
Communication Process
- Sender: conveys a message, encoded.
- Message: what is being conveyed.
- Receiver: listener, observer, decoder, to whom the message is for.
- Response/feedback: message returned to the sender, allowing correction or reward.
Modes of Communication
- Verbal communication: spoken or written word.
- Non-verbal communication: gestures, facial expressions, and touch, also known as "body language".
Documentation
- The process of preparing a complete record of a patient's care.
- Accurate, detailed charting shows the extent and quality of care provided, treatment, and education.
Types of Documentation
