Assessing Abilities and Capacities: Cognition Part 2
- Cognitive assessments should include patient/family self-reports, measures of specific cognitive domains/processes, and observations of functional performance.
- Occupational therapists may conduct all or part of the assessments, depending on the setting.
Self-Report Measures
- Standardized interviews and questionnaires are used to assess cognitive functioning, as rated by the patient.
- Individuals with impaired self-awareness might downplay problems in their self-reports.
- Discrepancies between a patient's self-report and that of significant others can indicate inaccurate self-perceptions.
Selected Measures Involving Self-Report
- Self-Awareness of Deficits Interview (SADI): This tool gathers both qualitative and quantitative data on self-awareness. The patient discusses their awareness of deficits, limitations, and ability to set realistic goals. A therapist scores the patient based on their functioning (0-3, 0 being full awareness). Administration time is 10 minutes.
- Behavioral Rating Inventory of Executive Function-Adult version (BRIEF-A): This is a standardized self-report and informant report questionnaire assessing executive functioning in everyday life. The BRIEF-A measures domains like inhibition, self-monitoring, planning, organization, initiation, task monitoring, emotional control, working memory, and material organization. The test has two broad indexes: behavioral regulation and metacognition. An overall summary score gives a global executive composite. Higher scores indicate greater difficulty. Administration time is 35 minutes.

Three Themes:
- Dynamic Assessment: Clinicians use cues and feedback to understand how to elicit optimal patient performance (Dynamic Interactional Assessment). Strategies and cues may be used to manipulate the task and environment for better performance.
- Informal Observation: Observations help therapists generate hypotheses about cognitive strengths and weaknesses, identifying areas for further evaluation.
- Performance-Based Assessment: Patients complete tasks that simulate everyday activities. The complexity, novelty, and challenge of the task are crucial elements when assessing executive abilities.
Performance-Based Measures
- Assessing executive abilities depends on adequate task complexity, novelty, and challenge. Tasks typically require problem-solving, managing distractions, and incorporating multitasking.
- Examples include Executive Function Performance Tests and the Multiple Errands Test.
Selected Measures of Functional Cognitive Performance
- Observed Tasks of Daily Living-Revised (OTDL-R): Assesses everyday problem-solving through tasks related to medication use, phone use, and financial management. Administration time is 25-30 minutes.
- Arnadottir OT-ADL Neurobehavioral Evaluation (A-ONE): This measure has two parts: Part 1 observes ADL performance and uses the Functional Independence Scale & Neurobehavioral Specific Impairment Subscale; Part 2 is an optional assessment to cross-check neurobehavioral impairment to likely lesion site. Administration time is 25 minutes.
- Executive Function Performance Test (EFPT): This standardized, performance-based measure evaluates executive functioning by assessing performance on 4 tasks related to self-maintenance (e.g., simple cooking, telephone use, medication management, bill paying). Administration time is 60 minutes.
Selected Cognitive Screens and Microbatteries
- Mini-Mental State Examination (MMSE): This screening tool includes 11 questions covering orientation, attention, learning, calculation, abstraction, information, construction, and delayed recall. Scores of 0-30 are used; scores of 24 or below may indicate potential cognitive disorder. Administration time is 5-10 minutes.
- Montreal Cognitive Assessment (MoCA): A screening tool for mild cognitive impairment. It assesses attention, concentration, executive functions, memory, language, constructional skills, conceptual thinking, calculation, and orientation. The MoCA can be a useful tool for patients experiencing memory difficulties who score within typical ranges on the MMSE. Administration time is 10-15 minutes.
- Loewenstein Occupational Therapy Cognitive Assessment (LOTCA): This microbattery consists of 20 subtests in four areas: orientation, perception, visuomotor operations, and thinking operations. Administration time is 30-45 minutes.
- Cognistat (Neurobehavioral Cognitive Status Examination): A microbattery of 10 subtests, including orientation, attention, comprehension, repetition, naming, construction, memory, calculation, similarities, and judgment. Administration time is 20-25 minutes.
Interpreting Results of Performance
- Neurobiological: Crystallized intelligence remains stable or improves in later life, while fluid intelligence declines. Visual/perceptual problems can affect memory input. Dehydration, anemia, and medications can influence cognitive performance.
- Affective: Anxiety, worry, and other stressors might consume working memory capacity. Depression can lead to disengagement and memory deficits. Transient mental distractions (e.g., pain, fatigue) can also affect executive function and working memory.
- Sociocultural: Cultural differences in information processing (objects vs. context) and educational background can influence performance on cognitive tests.
- Task & Environment: Familiarity of the task and environment similarity influence performance. Contextual cues and environmental factors (lighting, noise) impact performance.
