Phone call: - Complete referral form, - Enter info GoHealth Queue - Enter in therapist schedule - Make chart, put form in folder Upon arrival: -Provide demographic and handbooks -Get copy of insurance card -Admit client from queue -Enter client data -Enter client program -Enter PICIS -OHCA Eligibility -Intake packet - Upon Intake completion, - Notify therapist - Provide documents to Nurse -Enter guarantors Nurse -Wait for therapist to complete assessment -Start forms: - Admission Order - Admission History - Nurse Assessment - Aims Dyskinesia -Teams: -Send info to prescriber as available, name/DOB -Complete physical assessment and medication status; questions -Contact provider via teams and call when ready Client talks with provider then parent if needed/requested via teams call -after provider call’s completed, give survey to parent/client to fill out in front lobby If Client Admitted: -Search client, remove jewelry, belts, shoestrings, etc… -Lock up belongings in locker, front -Start Nurse Pprogress Notes -Finish all other forms @ end of shift do final nurse progress note and report to oncoming nurse -Place all documents in designated mailbox -When client discharges, discharge meds YES Tulsa Workflow - Front Desk/Office Specialist -Answer Telephones -Client Intake -Administrative Duties Therapist -OHCA verification (if FD/OS is not present) Guardian: - LMHP statement/crisis assessment -Disclosure of Professional Competencies Client : -LMHP statement/crisis assessment -PHQ - A -Columbia in -ACE questionnaire - Safety plan - Sign, review, discuss, & give copy of safety plan to guardian -Disclosure of Professional Competencies if able to sign Therapist: - Notify nurse & DC Coordinator LSC - Phones and Intake when FD/OS is not available - Upon completion of intake -NOMS -Take client photo - Sanctuary community meeting - Activities in Milieu - Snacks and meals - Intervention Progress Note - If admitted, assist with body search - Begin observation -LSC Duties checklist Walk-in: -Complete referral form, demographics & handbooks -Enter client into schedule -Notify therapy -Enter info in queue -Admit from queue to inpatient client -Get copy of insurance card -Admit client from queue -Enter client data -Enter client program -Enter PICIS -OHCA Eligibility -Intake packet - Upon Intake completion, - Notify therapist - Provide documents to Nurse -Enter guarantors Discharge Coordinator -Complete ROI if needed Coordinate/Schedule inpatient or outpatient services. -Complete warm hand off -27 CDC or 42CDC -Switch client to outpatient -Transfer client chart -24 hr follow up with client/guardian Data Specialist: - Retrieve all documents every morning from designated mailbox -Scan client documents into Gohealth -Prepare employee audits - Enter NOMS to SPARS -Fill out billing log for monthly reporting to accounting -Audit Dr. notes for provider completion each week -Prepare bi-monthly receipts log -Update CCBHC outcome measures each month -Update St. Francis and “by the numbers” data daily - Monitor and report HR Orientations/Relias Trainings bi-weekly
