MST Review/Hospital Acquired Infections Lecture 21 – Workshop 4 Dr Sacha Pidot, Dr Mark Davies By the end of this lecture you should be able to: To understand the difference between colonization and infection To understand how hospital pathogens can be tracked, traced and identified MST Review Overall results: Score (/35) % Average 29 85 SD 3.5 10.1 Median 31 88.6 100% of the class passed ~3/4 scored >80% MST Review Qs that were answered well – Most questions! – Diagnostic tests – Koch’s postulates – Vaccines MST Review Qs that were not answered so well – Choose the INCORRECT answer… Make sure you read the question properly! – Just select an answer Some questions left unanswered – why? MST Review Qs that were not answered so well MST Review Qs that were not answered so well MST Review Short answer questions Do: – Read the question! Four separate questions in this case Don’t: – Brain dump is not a good answer MST Review Short answer questions – Characteristics of high scoring answers Clear explanation of the required mechanisms Clear explanation of your opinion backed up by facts Description that shows you understood both the question and the response – Characteristics of low scoring answers Limited information, very short responses (1-2 lines) MST 2 Fri, 3rd May, 10am (usual lecture timeslot) – 45’ in length – Total of 35 marks – Worth 20% of final mark for subject Online quiz in LMS (as for MST 1) Short and extended answer Qs – No MCQs Covers lectures 14-26 Hospital Acquired Infections Paper 1: What is going on in this paper? – Colonisation vs infection – what do we mean? Hospital Acquired Infections How did they perform the study? Hospital Acquired Infections What did the authors find? Hospital Acquired Infections What are the limitations? Quality of individual studies Differences in surveillance methods Regional differences in data sources Limited evidence of clonal relationship between colonizing and infecting strains Etc… Hospital Acquired Infections Paper 2: Study questions: MDR S. epidermidis is a potential problem in hospitals – how much of a problem? What is the mechanism of resistance (esp to Vanc) in S. epidermidis? Clonal expansion of MDR S. epi in a hospital Hospital Acquired Infections Scenario: You have just received multiple bacterial isolates from patients with a serious skin infection. The patient samples were taken from inpatients in the ICU and renal wards, and several isolates have just come in from the transplant ward. The organism is Gram positive, but not normally known as a skin pathogen. Your job is to try to determine the origins, risk factors and potential for a hospital outbreak of this pathogen. This will help in understanding how to effectively treat and prevent further infections. Questions: How would you track the spread of this organism within the hospital? How could you determine if the organism had a hospital or community origin? If spread within the hospital is occurring, what factors might be involved in spreading?
