**[Osgood-Schlatter ]** ----------------------------------- Causes knee pain/swelling in adolescents due to growth spurt. Below the knee joint at tibial tuberosity growth plate (where patella meets top of shinbone) ### **[Anatomy]** Damages the growth plate at tibial tuberosity as well as the patella tendon. - Causing irregular bone growth - Swelling - Lower mobility ### **[Predisposing factors]** - **Growth spurt during adolescence** - **Repeated contraction of quadricep muscle** (weakens ossification centre.) - **Repeated forced knee extension** (repeated straightening of knee beyond **natural rom**.) - **Tight surrounding muscles (quadriceps)** put increased tension on patellar tendon, aggravated by exercise during sports with lots of **running** and **jumping**. - **Male gender** - **Excessive subtalar pronation** ### **[Mechanism]** - Excessive pulling on **tibial tuberosity** by **patella tendon** during **growth spurts in adolescence** which causes **swelling below the knee**. ### **[Treatment]** - **Reduced load** - Use of **orthotics** (limits subtalar pronation and stress on knee ligaments.) - **Sport withdrawal** - **Program modification** Stretching program - **Quadriceps stretch** (3x30s) - **Seated calf stretch** (5x30s) - **Knee straightened** (3 per hour) - Massage therapy - **Focused on quads** Muscle strength exercises (if possible)
