In order to maintain the highest possible standard Promote a positive air safety culture within the company, this includes: i\. Delivery of Flight Safety publicity and education IOT continue the development of a safe, secure, and professional approach to Air Safety (AS). ii\. Ensuring that the company complies with MAA, JHC and 1AvnX (Attk) Air Safety Policy. iii\. Prepare the unit for all Air Safety Audits and Inspections. Promoting and maintaining a positive safety reporting culture within the company i\. Management of DASORs and In-Forms submitted by Unit personnel and against the Unit. Ensuring that reports are actioned accordingly to ensure they are managed through to DDH review in the mandated time scales. ii\. Provide a 12 Monthly DASOR trend analysis to the 1AvnX Air Safety Team (AST) identify trends and offer mitigations as appropriate. iii\. Ensure that access to suitable methods of reporting is available to all exercising troops. Flight safety training and meetings: i\. Ensure that Company personnel are suitably qualified for respective posts. Ensuring Human Factors qualifications are up to date and POC for Air Safety related courses. ii\. Liaison with Bn Air Safety and Safety, Health, and Environment personnel to ensure that activities are conducted iaw with MAA and DSA regulations. iii\. Attend Station Flight Safety meetings and report outcomes back to the unit as and when necessary. -How are in forms actually reported? - there are multiple drop boxes around camp, e.g the scoff house (tends to be the more populated areas) ***[Cultures;]*** -Manual of Air Safety - Just- An atmosphere of trust where people are encouraged, and even rewarded, for providing Safety related information and where it is clear to everyone what is acceptable and unacceptable behaviour. It is not a blame free culture - Questioning- A culture where people are engaged and ready to ask, \"what if?\" and \"why?\" questions that challenge assumptions and reduce the possibility of incubated mistakes. - Learning- Organizational willingness and Competence to draw the right conclusions from its Safety information and to take appropriate actions based upon those conclusions. - Flexible- An organization that ►actively◄ adapts to changing circumstances and demands ►in a system of continuous improvement, working proactively to update Safety practices in response to changes in Risk / Hazards. ◄ - Reporting- An organizational climate where people readily report problems, errors and near misses. ***[RECORDING OF ENGINEERING AUTHORS;]*** -Who holds Auth level K & J - CO & OC respectively -Who holds level J and can give you auths? - OC & ASM \- What can level K do? - Give the OC auths -Which takes priority, the auths that are on stars or the hard copies in the office? - stars -What's important to check on stars regarding your auths? -- Platform and limitations to that auths -Which is more current/ more up to date? Ie sequence of searches (MAM P) when the auth level J is required to sign off the aircraft for unrestricted flight, what would they need to know before they sign it off? - -What is it thats missing? is it big, small, hard material etc -Where abouts on the aircraft was it last seen? (eg was it near critical components, moving parts etc) -What is the likelihood that it's on the aircraft? -Sequence: - Initial - Follow up - Deeper - X-ray/NDT - Airbourne: J980( oc,asm, aeo, qman), Lets the a/c go for restricted or unrestricted flight ***[NARRATIVE FAULT REPORT:]*** -760: - Monitors performance of new equipment/ equipment already fitted - Validity of current equipment - Monitors effectiveness of MOD'S -5V- you get the lists of components that need a 760 \- A 731 also goes with a 760 -They also go to the DT for trend Analysis -AEO,OC,ASM,QMAN, will sign of 760/ tell us to do one for the component ***[UNSATISFACTORY FEATURE REPORT:]*** -765 -Problems with TI's such as RTI,UTI etc either spellings mistakes or procedural mistakes -IETM -Enquiries on resolve are there for if the TI doesn't state damage limits etc it might be on there. There is also extra information for certain jobs on there. ***[CAMO:]*** -Continuing Airworthiness Management Organisation An organisation dedicated to ensuring the continuing airworthiness management of the air system. #### - ***The 1AvnX (Attack) CAMO is to ensure AH-64E Continuing Airworthiness in support of the Deputy Commander's Missions and Tasks.*** -Mil CAM is in charge of CAMO -Annual checks: MAR (Military Airworthiness Review) This is conducted by the MiL CAM where a group of people will come and check the Aircraft\'s Airworthiness, Serialised parts/components, Cleanness of A/C etc... -After the MAR is done we get a MARC (MOD F 710) which is a certificate that goes in the back of the 700C and the A/C can\'t operate without it. -9 Airworthiness tasks: A. Develop and control an Aircraft Maintenance Programme (AMP), support any applicable reliability programme and propose amendments and additions to the Maintenance schedule to the Type Airworthiness Authority (TAA) (Develop and control an AMP and support any applicable programme and propose amendments and additions to the maintenance schedule to the TAA) b\. Manage the embodiment of Modifications and Repairs. (Manage MODS and Repairs) c\. Ensure that all Maintenance is carried out to the required Quality and in accordance with (iaw) the AMP, and correctly released (Ensure maintenance is carried out to the required quality in the accordance with AMP, and correctly released) d\. Ensure that all applicable Special Instructions (Technical) (SI(T)) have their requirements fulfilled. e\. Ensure that Military Maintenance Organizations (MMOs) and / or MRP Part 145 Approved Maintenance Organizations (AMOs) correctly manage Faults reported, or discovered, during scheduled Maintenance (Ensure MMO and or MRP AMOs correctly manage faults reported, discovered during sched main) f\. Co-ordinate scheduled Maintenance, fulfilling the requirements of SI(T)s and the replacement of service life limited parts. g\. Manage and archive all Continuing Airworthiness (CAw) records and the operator\'s technical log. h\. Conduct Assurance that the weight and moment statement reflect the current status of the Aircraft. i\. Initiate and coordinate any necessary actions and follow-up activity highlighted by an Occurrence report. -They give us: - Techs to see - RTI's, UTI's, Procedures etc... and also Deviations from the DT - Makes sure that all TI's are up-to-date - looks at DASOR'S/DACOR'S -Works with DDH to make sure the APACHE fleet is at most available whilst keeping air safety -- I think they might also audit us?????????? ***[DT, ILOC AND FSR:]*** -.If a problem with a system occurred and a field service representative was called out. They told you what to do but it couldn\'t be found on the IETM. Can you take their word for it and carry out the job? - No, they can only give advice, they have no authority. It would have to be raised and you would have to wait for further instruction from the DT. \- What are FSR? Field service representatives. \- what do they do? -- Gives useful insight into components/systems that can allow us to repair the Air System -DT: Delivery Team: - Manufactures - any deviation needed from ietm comes from them - Managed trend analysis from 760's etc - Supplies/ gives us the necessary tools - Makes TI's -ILOC: Integrated Logistics Operations Centre: - spares support - manage spares - Sources spares [ **TOR'S**] a\.
