Paraphilic Disorders Repeated & intense sexual urges, fantasies, or behaviors in response to objects or situations that society deems inappropriate May behave inappropriately May involve multiple paraphilia displays Few people receive a formal diagnosis but patterns may be common CAUTION! How is a paraphilic disorder diagnosed?  For at least, the person experiences recurrent and intense sexually arousing fantasies, urges, or behaviors that involve objects or situations outside the usual sexual norms o Nonhuman objects o nongenital body parts o suffering or humiliation of oneself or one’s partner o children or other nonconsenting persons o Experience significant distress or impairment over the fantasies, urges, or behaviors Etiology and Treatment for these disorders? o Little formal evidence to support theories o No treatments received much research or proven clearly effective o Psychological, sociocultural, biological interventions used 02 Review of the different types of paraphilic disorders Fetishistic Disorder Key Features Recurrent intense sexual urges or fantasies involving Theories Psychodynamic: Behaviorist: Cognitive-behavioral treatment Aversion therapy, covert sensitization, imagined aversive stimulation, masturbatory satiation Transvestic disorder (transvestism, cross-dressing) o fantasies, urges, or behaviors involving o Heterosexual male began during childhood or adolescence o Does not involve transgender feelings or behaviors o Cognitive behaviorist theory: learning through classical conditioning early in life Exhibitionistic Disorder o Arousal from the o sexual contact rarely initiated or desired o Usually starts before 18 o Common among males o Theories of etiology: immaturity in interpersonal and sexual relationships; fears about masculinity; possessive mother o Treatment: aversion therapy, masturbatory satiation, social skills training, insight therapy Voyeuristic Disorder o repeated and intense sexual urges o Usually begins before age 15 o Invasion of other people’s privacy o Theories:  Psychodynamic – feelings of inadequacy or shyness drive the search for power  Cognitive-behaviorist – learned behavior traced to a chance and secret observation of a sexually arousing scene Frotteuristic Disorder Recurrent and intense fantasies, urges, or behaviors involving Begins in adolescence or earlier Pedophilic Disorder repeated and intense sexual urges or fantasies about watching, touching, or engaging in sexual acts with children, and either act on these urges or experiences result in clinically significant distress or impairment 2/3 of child victims are girls Pedophilic Disorder  Emerge during adolescence  may have been sexually abused as children  immature, display distorted thinking, another psychological disorder  If married, have sexual difficulties  Often imprisoned or forced into treatment if caught  Treatments  Aversion therapy, masturbatory satiation, antiandrogen drugs  Cognitive-behavioral treatment (relapse-prevention training) Sexual Masochism Disorder o fantasies, urges, or behaviors involving the act or the thought of o hypoxyphilia; autoerotic asphyxia o Etiology o start in childhood o develop through behavioral process of classical conditioning Sexual Sadism Disorder o repeated, intense sexual arousal from fantasies, urges, or behaviors involving thought or act of o Theories: o Cognitive-behavioral – classical conditioning and modeling (Internet sex sites, sexual videos, magazines, books) o Cognitive-behavioral & psychodynamic – sense of sexual inadequacy drives need to inflict pain o Biological – brain and hormonal abnormalities o Treatment o Aversion therapy, masturbatory satiation, CBT, antiandrogen drugs, relapse-prevention training Salamat!
