Summary

This document describes and illustrates different postures and procedures for manipulating the scapula, including lateral-to-medial and medial-to-lateral glides. Instructions on proper patient positioning and hand placement are included, along with vectors and location of pressure points for specific procedures.

SCAPULOCOSTAL SIDE POSTURE: Bimanual thumb thenar/lateral scapula; lateral-to-medial glide (Figure 6-72) DP: Square to scissor stance facing the pa ent. PP: Side lying (perpendicular to the table) involved side up guarding the breast ssue with contralateral hand. Ipsilateral arm along the side of their body (involved side up). Headpiece up or pa ent’s head supported with a pillow and ankle piece extended to support the pa ent’s ankles. CP: Bilateral thumb pads (make a field goal with thumb pads on lateral border of scapula SCP: Lateral border of the scapula Vector/LOD: Lateral to medial P: Trac on scapula L-M removing the so ssue slack, at the elas c barrier thrust/impulse L-M. Crossed bilateral mid-hypothenar (knife-edge)/scapula; medial-to-lateral glide (Figure 6-73) DP: Square to scissor stance facing the pa ent PP: Side lying (perpendicular to the table) involved side up guarding the breast ssue with contralateral hand. Ipsilateral arm hanging forward off the edge of the table (involved side up). They may look like they are face down, but you must begin in side posture. CP: Inferior Calcaneal (keep elbow down) SCP: Medial (vertebral) border of the scapula Vector/LOD: M-L P: Ask pt to lay on their side facing you. Roll the pa ent towards you so that their affected arm is hanging off the front of the table. Stabilize the uninvolved scapula towards the table with superior calcaneal while trac oning involved scapula M-L with inferior calcaneal (lean towards the pa ent and twist) and thrust/impulse M-L at the elas c barrier. Bimanual digital thenar grasp/scapula; rota on—inferior angle lateral to medial (Figure 6-74) DP: Square to scissor stance facing the pa ent. PP: Side lying (perpendicular to the table) involved side up guarding the breast ssue with contralateral hand. Move involved arm from the pa ent’s side: ask to make a fist and place it on their lower back (shoulder abduc on). This ini ates the lateral to medial mo on of the inferior pole of the scapula. CP: Inferior hand web contact (thumb on lateral border and index on medial border) IH: Cephalic hand grasps spine of the involved scapula SCP: Inferior pole of the involved scapula Vector/LOD: L-M at inferior pole of the scapula thrust with inferior hand with slight pull or M-L pressure with cephalic hand to induce rota on of the scapula in the correct direc on and impulse/thrust (mostly L-M on inferior pole). P: Use both hands to create a rota onal, twis ng mo on, using an impulse type thrust driving the inferior pole L-M Bimanual digital thenar grasp/scapula; rota on—inferior angle medial to lateral (Figure 6-75) DP: Square to scissor stance facing the pa ent PP: Side lying (perpendicular to the table) involved side up guarding the breast ssue with contralateral hand. Move involved arm from the pa ent’s side: ask place their hand (involved side) on the back of the head to begin the M-L pretension on the inferior pole of the scapula. CP: Inferior hand web contact (thumb on lateral border and index on medial border) IH: Cephalic hand grasps spine of the involved scapula SCP: Inferior pole of the involved scapula Vector/LOD: M-L at inferior pole of the scapula thrust with inferior hand with slight pull or M-L pressure with cephalic hand to induce rota on of the scapula in the correct direc on and impulse/thrust (mostly L-M on inferior pole). P: Use both hands to create a rota onal, twis ng mo on, using an impulse type thrust driving the inferior pole L-M
