PROFESSIONAL OVERSIGHT – JOB PERFORMANCE INTERVIEW AND RECOGNITION POLICY: It is the policy of the Omaha Police Department (OPD) that sworn supervisors will utilize a Job Performance Interview (JPI) Form (OPD Form 13) when they interview a sworn employee about the employee’s unacceptable job performance and/or failure to follow specific job instructions. A JPI is not discipline. OPD sworn supervisors will similarly utilize a Job Performance Recognition (JPR) Form (OPD Form 13A) to document positive job actions and performance. PROCEDURE: I. Job Performance Interview (JPI) A. Purpose 1. A JPI (OPD Form 13) will document unacceptable job performance and/or failure to follow specific job instructions. a. This form provides written proof that counseling has taken place and that the employee is aware of the problem. - AND - b. This form provides a quick method of recalling details of prior poor performance and thus allows the supervisor to testify from facts. B. Duties and Responsibilities 1. It is the supervisor’s responsibility to monitor the work performance of subordinates. When work-related problems arise a supervisors will take appropriate and timely corrective action. 2. The most appropriate course of action is to discuss (counsel) the problem with the employee. These discussions should be documented using a JPI (OPD Form 13). 3. The JPI shall be specific, accurate, factual, and identify proposed solutions. a. If reported by another person, the supervisor should record this on the form and if possible attach any written communication. NOTE: If the Pursuit Review Committee (PRC), Safety Review Board (SRB), or Internal Affairs (IA), directs a supervisor to issue a JPI, additional documentation is not required. b. Record observations of the employee’s behavior. c. Document the employee’s explanation. d. Document the employee’s suggestions for improvement. e. Identify and document a specific and agreed upon plan for improvement. NOTE: Make sure the employee understands the department’s expectations moving forward. f. The employee will sign OPD Form 13 at the completion of the interview. (1) Employees who refuse to sign OPD Form 13 commit “Disobedience of Orders,” per the OPD “Rules of Conduct” policy. 4. Routing of Completed JPI (OPD Form 13) a. The original form will be forwarded to the Professional Oversight Bureau (POB) Deputy Chief. The POB Deputy Chief will document the form in the employee’s IAPRO file and then forward to the OPD Personnel Unit. b. The original form will be maintained in the employee’s OPD Personnel Unit File at OPD Central Headquarters. c. A copy of the form will be provided to the employee. d. No other copies of the completed form will be distributed. e. Any JPI issued to an employee that is over one (1) year old shall not be used as a basis to show any type of behavioral or performance pattern. f. It is the responsibility of the employee to request the removal of a JPI that is older than one year old from the date of issuance, in accordance with the Collective Bargaining Agreement between the Omaha Police Officers’ Association (OPOA) and the City of Omaha. II. Job Performance Recognition (JPR) A. Purpose 1. A JPR (OPD Form 13A) will document positive work-related performance issues or to document positive public feedback when the positive work-related performance does not rise to the level of formal recognition. B. Duties and Responsibilities 1. The JPR shall be specific, accurate, factual, and identify the positive job performance. a. Record observations of the employee’s behavior. b. If reported by another person, the supervisor should record this on the form and if possible attach any written communication. 2. Routing of Completed JPR (OPD Form 13A) a. The original form will be forwarded to the Executive Services Bureau (ESB) Deputy Chief via the employee’s chain of command. The POB Deputy Chief will document the form in the employee’s IAPRO file and then forward to the OPD Personnel Unit. b. The original form will be maintained in the employee’s OPD Personnel Unit File at OPD Central Headquarters. c. A copy will be placed in the employee’s command file. d.
