# PERFORMANCE APPRAISAL: QUARTERLY EVALUATION FOR ENTRY-LEVEL PROBATIONARY EMPLOYEES ## POLICY: It will be the policy of the Omaha Police Department to establish and maintain a system for employee evaluations. ## PROCEDURE: ### **1. Performance Evaluation for Entry-Level Probationary Employees** * **A. An in-depth evaluation of an employee's job performance during their probationary period will be conducted quarterly. The Omaha Police Department, Quarterly Performance Review form will be used for documentation. Such evaluation should include the following issues and observations.** * 1. Specific examples of job performance * 2. Current level of development * 3. Work attitude * 4. Quality and volume of work * 5. Judgment * 6. Other appropriate indicators of performance appropriate to the job * **B. Evaluation processes for probationary police officers will be conducted as follows:** * 1. During Academy Training: The Training Unit will continuously evaluate performance based upon test performance, inspections, and evaluation of conduct. Written feedback will be provided for critical incidents and substandard performance. * **C. During Field Training and Evaluation Program the Field Training Officer becomes the Recruit Officer's "immediate supervisor" for training and evaluation purposes. During this period, the FTO will complete a Daily Observation Report.** * **D. Following the Field Training Program: During the balance of the probationary period, the Sergeant who supervises the Probationary Officer will utilize the Quarterly Performance Review Form to evaluate performance quarterly. This report will be approved by and forwarded to the Precinct Commander.** * **E. Non-sworn, entry-level, probationary employees will receive a quarterly performance evaluation based on criteria outlined in I.A.** * **F. Reports will be completed, using the Omaha Police Department Quarterly Performance Review form OPD Form 153, by the first level supervisor and reviewed, and forwarded to the Unit Commander.** * 1.
