FR-A-104 Employee Performance Review - List of annual and 12-month probationary period for Employee Performance Reviews forwarded to the employee's DC or Section Manager the beginning of every month. - All employee Performance Review forms located in "My Evaluations" on Target Solutions. - Responsible for ensuring completed and submitted on time... DC or Section Manager. - Defined as person directly overseeing the employee, responsible for all members at assigned fire station... Supervisor/ Operational Captain. - LT's shall be consulted for employee's they directly oversee. - Minimum of [3 goals] identified in the "Recommended Attainable Goals for Employee Next Rating Period" section. - Positive and negative feedback should be given throughout the year in accordance of policies... Professional development and Employee Development. - Employee signature required, does not necessarily imply they agree with review but rather that they received and reviewed it. - The Supervisor will then email the signed Employee Performance Review to the appropriate next-level supervisor utilizing the email function within the "My Evaluations" section of Target Solutions. - Supervisors shall not alter any Employee Performance Review after the employee reviews and signs it. - If employee feels supervisor incorrectly rated them, can take to the next level supervisor, however they may not change the rating. - Grievance may be filed but not taken to arbitration unless it involves a rating of unacceptable or needs improvement. - These rating's require complete documentation which shall be attached as well as a sub-sequent Special Employee Performance Review in 4 months, notification to Battalion Command Staff or supervisory staff and notification to payroll. - Once the Supervisor, employee, and the appropriate next-level supervisor have all signed the Employee Performance Review, it shall be considered complete and will remain in the employees file in "My Evaluations" on Target Solutions. - Permanent status bargaining unit personnel have annual Employee Performance Review on or before anniversary of hire or promotion, receive step on this date and if do not rate above unacceptable or needs improvement step rescinded, must payback in time frame it was received, and does not receive until performance is improved above unacceptable. - Must work at least [4 months] to have a performance review for that year, if not done, will not receive annual step raise. - Once returned to work and works for 4-months, can receive employee performance review rating of "acceptable" and be retro paid step to the return to work date. - Utilized when employee does not correct an identified problem after counseling and/or disciplinary action... Special Employee Performance review. - Employee's unable to raise "needs improvement" or "unacceptable" rating in 4- months to an "acceptable" rating, may face loss of assignment or recommendation for termination. - Probationary firefighters receive reviews following... completion of recruit academy, 6 months continuous service, 8 months and 11 months. - Submitted to payroll two weeks prior to probation ending, after recommendation of supervisor for permanent status or termination. - Promotions, reviews should be done in [4 months] and conclude a recommendation of permanent status or extension of 2-month probation. - Annual reviews shall now be done on the anniversary date of employee's promotion. - If on Special assignment for 6 months or longer, Special Performance Review shall be completed by supervisor before returning to permanent position. FR-A-105 Employee Development - A non-disciplinary form utilized to document negative Fire Rescue personnel performance as a method of coaching and counseling... FR-A-108 Workplace Violence Prevention and Response - Workplace violence is outlined in 5 parts: Prohibited weapons, Prohibited Behaviors, Identifiable Characteristics, Warning Signs, Response to act or threats of violence. - Prohibited weapons must refer to PBC Policy Weapons in Governmental Workplace for weapons on County Property. - Prohibited Behaviors are threats of violence against persons or property that are sever, offensive, threatening, or intimidating to alter employee conditions or to create a hostile, abusive, or intimidating work environment for any Fire Rescue Personnel or reservist. - Considered acts or threats of violence: - Hitting, kicking, punching, slapping, shoving. - Threatening harm to individual or family, friends, associated or damage their property. - Intentional destruction or threat of destruction or property owned, operated or controlled by Fire Rescue. - Harassing or threatening telephone calls, letters or other forms of written/electronic communication. - Use of foul language directed at another person in a threatening or hostile manner. - Intimidating or attempting to coerce a Fire Rescue personnel to do wrongful acts. - Harassing surveillance or stalking. - Identifiable Characteristics: 35-50 y/o male, prefers own company, fascination with weapons, few outside interests, equated job to self-esteem, history of interpersonal conflict, sense of being entitled like owed a debt, extreme views/ paranoid behavior/ difficulty accepting criticism, stress such as health or family trouble, unwelcome comments or behaviors at work related to sex or violence, perceived danger of discipline, layoff, or other negative event at work. - As the number of characteristics increase, supervisors should maintain a high level of awareness for below warning signs. - Warning signs: threats to self or others, inappropriate anger and rage, despondence, persistent and exaggerated perceptions of injustice, erratic behavior, violence against family member, obsessive harassment and stalking, paranoia and delusions, substance abuse. - Any personnel observing these warning signs of a Fire Rescue member should report to the next level supervisor and continued until reaching minimum level of Deputy Chief. - Non-imminent threat or danger. - If it is a supervisor conducting such behavior, reported to next level supervisor above them. - Immediately forwarded to minimum level Deputy Chief and shall include notifications to the IA Officer and Union officials. - Member may be relieved of duty pending investigation and access to all fire rescue facilities restricted. - If employee is leaving station or already off duty, Law Enforcement shall be notified to go to their home or last known location to make an evaluation and act accordingly. - Following actions taken: swipe card deactivated, Notice of Formal Investigation (NOFI) access to staffing software and e-mail restricted, mandatory EAP referral. - When returning back to full duty, EMS Captain or BC assigned to run calls with employee to determine "fit for duty" and that conduct is becoming of Fire Rescue employee. - Imminent Threat or Danger: immediately make 911 call and secondly and when safe to do so notify FOO or communications center. - FOO will make chain-of-command supervisory notifications to EMS, BC, DC, Division, and appropriate Deputy, then notifications to IA and Union officials. - Actions taken: work with LE and unified command, provide information to employees on what to do if they go to the workplace, "lock-down" based on threat assessment and routine police patrol of fire stations and work locations, target victims identified to LE and address flagged for patrols, targeted work locations have personnel relocated, accused relieved of duty and all other "Non-Imminent" items followed from above. FR-A-109 Reportable Event - Two Categories for reportable events... Immediately Reportable Event (IRE) and Delayed Reportable Event (DRE). - IRE examples... action or inaction caused harm to citizen, pt, or FD personnel; equipment failure that caused harm; on-duty event that is illegal, unethical or immoral or any accusation thereof; any event having potential for litigation against FD; and any event that is or has potential to be political. - Must notify supervisor ASAP without compromising the safety of FD personnel, pt or the public. - By phone or face-to-face to not cause delay and forwarded up the chain of command to the BC (If supervisors unavailable, including BC, contact the FOO for closest available BC). - BC may place the unit OOS and may remain at the incident unless concern for safety. - Leave all items as undisturbed as practical until the IRE is investigated and pictures can be taken. - Preliminary investigation of the IRE site and personnel done by... Chief of Ops, DC's, IA Officer, Litigation Officer, EMS and Fire CQI's, and the Safety Committee. - Div of Ops will initiate meeting with Deputy of Ops and involved supervisory staff to ascertain the need for further investigation such as... additional investigation, Formal Investigation, CQI Analysis, safety Committee investigation, criminal investigation or required notifications to the State EMS Office. - DRE treated the same as IRE with the exception that... FR-A-201 Professional Appearance Standards - Has the sole right to determine whether or not FD personnel has met the professional appearance standard, and shall have the right to direct personnel to correct problems with their appearance at the time of discovery by a supervisor... FR-A-206 Reporting Absence from Duty - Report to duty in uniform, scheduled station, gear on proper truck prior to 0730. - Recruit academy, 0730 unless otherwise directed by Training and Safety Division. - Training and Safety division 0800 hrs for 40-hour work week and may alter report time and/or hours in order to accommodate scheduled training functions occurring that day. - Admin 0800-1700 may elect alternate schedule, varying starting and ending times, but must be reasonable and adhering to industry standards. - Require immediate supervisor's approval. - Failure to report an absence within identified time limits either reporting to duty late or AWOL. - Absence from duty may include... approved sick leave, approved funeral leave or requested emergency leave. - Report of absence should be done at least [30 minutes] prior to shift on staffing system or calling the FOO. - If FOO is not available, leave message on voicemail with\... name, Battalion/assignment, reason for absence, and name of supervisor. - Aviation battalion should notify the on0duty ARFF Captain by 0700 hrs. - Special Event, notify FOO 2 hours before schooled start. - FOO will notify Special Events Coordinator and is their responsibility to fill the position. - Recruit academy report absence to Training and Safety Personnel. - Day's personnel report absence no later than 30 minutes after designated start time to supervisor/ section manager. - Bargaining unit personnel, for approval of Emergency leave, must speak directly to... FR-A-404 Use of Social Media - This policy applies to all Palm Beach County Fire Rescue personnel and reservists, whether their social networking activity is conducted in or outside the workplace, while on or off duty, anonymously, or through the use of pseudonyms. - Harassment: The systematic or continued unwelcome actions of one party or group intended to demean, threaten, intimidate, or alarm another party or group. - Protected concerted activity: Employees acting for mutual aid and protection regarding their terms, pay, or working conditions of employment. - Public Information Office and staff assigned by the Deputy Chief of Administration shall help promote authorized social media accounts. - Employees are prohibited from accessing social media for personal purposes through County technology systems. - Personnel are cautioned against posting or displaying information containing Department logos, uniforms, vehicles or facilities, if the associated content or context reflects poorly on the Department, or the community's perception of the Department. - Personnel are cautioned not to display or post personal photographs, videos, images, or provide similar means of personal recognition as a firefighter, fire officer, or employee of PBCFR, if the associated content or context reflects poorly on the Department or the community's perception of the Department. - Failure to comply with this Use of Social Media PPM may result in discipline up to and including termination. - Fire Rescue personnel are strongly encouraged to use caution and seek the guidance of their supervisors regarding any posting that may be prohibited pursuant to this PPM. FR-B-101 Investigator Call Out - Violation of any federal, state, local, statue, law, ordinance, rule or regulation... [Crime-Related]. - Any incident or event where either a civilian, employee or FF of FD may or may not require transport to a medical health facility and has no signs of life or have been declared as deceased... [Loss of Life.] - Any incident or event where either a civilian, employee or FF od FD may or may not require transport to a medical health facility and has incurred an injury at any location as a result of a fire or burn and requires any type of medical treatment... [Casualty.] - Any incident or event in which a person under 18 has a direct involvement in the ignition of the incident or event, may or may not include physically being present at the location or by any eyewitness identification... [Juvenile-Related Incident.] - Any device that relies on the exothermic reaction of an explosive material to provide a violent release of energy, any improvised incendiary device commonly referred to as a Molotov Cocktail, any fireworks, pyrotechnic devices, or flame effect... [Explosive Devices. ] - IC call-out of Investigator when there has been a fire or explosion incident or event meeting this criteria: - Evidence of crime-related or foul play regardless of dollar value. - Loss of life/injury (injury includes ALS treatment or transport). - FF Loss of Life or Injury (ensemble collected by LODD team and kept in PBCFR Evidence Room). - This investigator call out requires response of 2 investigators. - Accidental Fires with substantial monetary loss or structural damage. - Loss exceeding \$70,000 or 20% of structure (does not include water/ smoke damage). - Juvenile Related Incidents (if direct involvement unknown, contact investigator and they will determine response). - Explosion device/ explosion (less than \$70,000 or 20% of structure, call investigator and they will determine response). - All other circumstances... cause unable to be determined by crews, any sprinkler or fire suppression activation when the circumstances for activation are for anything other than accidental causes or potential for high value loss, any overcrowding/ potential for high value loss or potential loss of life exists, and if IC determines significant life safety issue exists. - Investigator request from IC through the Alarm Office, along with PD having jurisdiction (not to assist with investigation but securing scene or crowd control) any time investigator is requested. - FD unit must stay on scene until investigator arrives to maintain scene custody. FR-B-105 Recreational, Ceremonial and other Non-Permit Fires - Ceremonial fires are bonfires and fires for religious purposes. - Recreational fires are campfires, cooking fires, or fires for warmth. - Backyard fires with the intent to dispose of yard debris is not permitted in jurisdiction of PBCFR. - Not required to have a permit or letter of approval... Recreational fires. - Need an approval letter form the Fire Marshal's Office...
