12/6/2023 Medical  Unless otherwise directed by the court, a medical certificate shall be signed by a physician or licensed psychologist and shall contain: Certificates  (1) a description of the nature, type, and extent of the person's specific cognitive and functional under Chapter  limitations; (2) an evaluation of the person's mental and 190B physical condition and, if appropriate, educational potential, adaptive behavior, and social skills;  (3) a prognosis for improvement and a (2022) ©GERALD P. KOOCHER recommendation as to the appropriate treatment or habilitation plan; and  (4) the date of any examination upon which the report is based.  A person alleged to have an intellectual disability shall be examined by a clinical team consisting of a physician, a licensed psychologist and a social worker, each of whom is experienced in the evaluation of persons with an intellectual disability, who shall report their conclusions to the Court. 32 Antipsychotic Medications Chapter 123: Section 8B.
