# Outcomes of NSPT A diagram illustrates the possible outcomes of NSPT (non-surgical periodontal therapy), categorized into: ## Benefits of NSPT - Increased chance of keeping teeth - Improvement in systemic/general health - Reduced pain opportunity - Financial impact long term due to modified risk factors - Positive health messages – create behaviour chance within family - Quality of life improvement ## AIMS of Periodontal Treatment - Easier for patient to clean and manage - Lower chance of biofilm accumulation - Have to address patients main concerns - Make patients aware - Minimising impact of non-modifiable risks - Raise concern - Modifiable risk factors ## Outcomes of NSPT - **Reduced plaque/bleeding:** Sign of active disease, <10% aim - **Pocket depth reduction:** Plaque primary aetiological cause of disease - **Avoid patient complaint:** Once a perio patient, always a perio patient - **Increased cleansability** : Easier for patient to clean and access - **Maintenance of OH:** Remove areas of increased plaque accumulation - **Different OH aids:** Naturally occurring or iatrogenic - **Reducing pocket depths:** Educate patient that long term maintenance is required - **Making the oral cavity easier to clean/manage:** No treatment is risk free! (Provide patient with written risks and benefits to gain informed consent) ## RISKS of Periodontal Treatment - **Understandable due to soft tissue manipulation:** Depends on inflammation levels - **Discomfort:** Normal pain relief advised - **Always a risk during NSPT:** Encourage to maintain OH - **Lost restoration:** Encourage to maintain OH - **Complicated disease:** Reassure patient - **Non-responding site:** Aesthetics - **Patient not responding to treatment:** Recession - **Black triangles:** Bleeding - **Aesthetics:** Sensitivity - **Not suitable for floss:** Inappropriate debridement (PMPR) - **Food trapping:** Recession/exposed dentine tubules - **Reduction of interdental papilla:** Affect quality of life - **Increased caries risk:** Sign of positive treatment - **Recession:** Dentine higher critical pH 6.5 ## Four Steps Recommended by BSP **1. Step 1** - Assessment - Education - Risk factors - OH coaching **2. Step 2** - OH, risks - PMPR, H&U - Adjunctive antimicrobials **3. Step 3** - Manage non-responding sites - Re-treat - Refer **4. Step 4** - Maintenance - Targeted PMPR - OH, Risk factor
