Duties and Responsibilities of the Pharmacist o In charge of the Pharmacy Section of SC Clinic Pharmacy o In charge of the License To Operate SC Clinic Pharmacy to Food and Administration o Monitors the latest updates required by the government agency (Department of Health) with regards to policy and procurement of medicines. o Provides the Philippine National Drug Formulary for all concerned staff in Medical Services o Prepares the list of medicines needed in the procurement plan o Check and File the Purchase Order from the Property Division o Monitors drug companies for deliveries and problems with expiry dates o Dispense medicines and instruct patients on their intake for all employees of the Court including staff in SC Tagaytay and SC Baguio o Responsible for the distribution of Vitamins to 54 offices in the Supreme Court o Dispensed all prescriptions and filled up the Requisition slip completely o Conduct Monthly Inventory twice a year with Accounting staff, COA and Property Division o Record all maintenance medicines in a Blue Book for accurate monitoring o Record all maintenance medicines in a Blue Book for accurate monitoring o Assist in the preparation of the yearly budget for medicines and supplies of the 2004 Pharmacy and Medical Clinic (Emergency). o Daily recording slips of dispensed medicines o Prepares reports of monthly Dispensing Report (Consolidated) of Medicines o Prepares monthly Reports on Supplies and Materials for Main Clinic o Prepares monthly report for the Number of patients served. o Prepare a List of Available Medicines for all Prescribing Doctors and Dentists o Prepares and leads the SC Clinic Pharmacy Programs SC MDS PHARMACY POLICY o Strictly implement the NO PATIENT, NO MEDICINE POLICY, except under extenuating circumstances. o Strictly implement the NO Prescription, No MEDICINE Policy o Substitution of Medicines is allowed only upon instructions of the prescribing doctor and with the consent of the patient. o Doubling of medicine dosage forms is not allowed. o Refill of medicine BY REPRESENTATIVE is NOT allowed, except under extenuating circumstances. o All medicines packs must have MEDICINES LABEIS which must be properly filled up by the pharmacists. o Appropriate Medication Review and Patient Medicine Counselling At the time of dispensing of medicines: Indication, Proper Use, Precautions and other relevant Information. o All dispensed medicines must be Tabulated Properly and Accurately, according to Brand name. Dosage forms and Quantity. o The pharmacists are responsible for the preparation of the consolidated periodic pharmacy reports, and submission of the medicine dispensation statistics completely and accurately. o Procuring medicine needs to follow the Generic ACT of 1988 (RA 6675) and the Government Procurement Act. o All deliveries must be received /counter-checked by the pharmacists versus the Purchase Request and Sales Invoice. o The storage and arrangement of medicines in the Medicines Stock Room is the sole responsibility of the pharmacists. o Copies of pertinent documents (including Patient Medication Profile Good Pharmacy Practice Code, Good Storage Practice Code, and Risk Management Plan as required by BFAD, must be readily available at all times. Social Security System - Suitable to the needs of the people throughout the Philippines which shall promote social justice through savings, and ensure meaningful social security protection to members and their beneficiaries. Pharmacy and Benefit Control o Assurance – assures that its inventory of medicines is well- stocked and maintained properly. o Monitoring – monitors the inventory purchased medicines by recording the following details: invoice, unit cost, expiration date. o Compliance – ensures compliance to government regulatory requirements as to LTO and deregulated license (S1) / Maintenance of the Prescription Book and Opium Additional Book
