Spleen Ezzatabadipour Massood Associate Professor KMU 2022 1 Spleen  It is the largest peripheral lymph organ  Like the thymus, it has only efferent lymph vessels  Its size is similar to each person`s closed fist (1, 3 and 5 inches) and weighing about 7 ounces (150 to 200 grams)  It is intraperitoneal organ which connects to stomach and left kidney by gastrosplenic and lienorenal ligaments  It is like a reservoir of blood 2 Spleen  It is a bloody organ with large vessels  The largest branch of the ciliac trunk (splenic artery) enters the spleen after passing through the back of the stomach and pancreas and the posterior wall of the lesser sac & splenorenal lig.  Located in the upper left part of the abdominal cavity (left hypochondrium area), which protected by left 9, 10 and 11 ribs (its long axis along 10th rib)  About half of monocytes, a quarter of lymphocytes and about the size of a cup (about 240 ml) reserve red blood cells 3 Spleen  It has two upper and lower borders and two posterior and anterior ends and two surfaces: diaphragmatic and visceral  The upper border has notches, superior border notched  There are two diaphragmatic (suproletral) and  visceral (infromedial) surfaces adjacent to the stomach, pancreas, left colic and left kidney  At the visceral surface, the hilum of spleen (adjacent to tail of pancreas) is located which close to the inferior border  The anterior end extends to the left mid-axillary line (adjacent to the phrenico-colic ligament)  The posterior end is at the level of T10 and 4 cm from the midline of the back  Due to the proximity of the spleen to the ribs, rib fractures may lead to damage and rupture of the spleen 4 Splenic artery  It the largest branch of the arterial celiac trunk  It has the most tortuous path in the body  In its path, first slightly down, then the horizontal path to the left and  It is placed first on the top of the neck and then on the back of the body and the tail of the pancreas 5 Short gastric and left gastroepiploic arteries  The short gastric and left gastroepiploic branches pass through the gastroesplenic ligament  The short gastric enter the gastric fundus & the left gastroepiploic branch to the great curvature of the stomach  The splenic artery, along with the tail of the pancreas, passes through the linorenal ligament 6 Splenic vein  It is one of the three main branches involved in the formation of port veins With the participation of two other veins (Superior & inferior mesenteric veins)  It also receives veins from the stomach, pancreas, and inferior mesenteric vein  Behind the neck of the pancreas and around the second lumbar vertebra and in front of the vena cava inferior, it connects to the superior mesenteric vein, creating portal vein 7
