STANDARD OPERATING GUIDELINE FAILURE TO RESPOND Date Issued: March 29, 2019 Rescinds: December 1, 2008 Section: Incident and Emergency Operations File Code: G-FAIL PURPOSE To provide all Toronto Fire Services personnel with a guideline for rare situations in which apparatus fail to respond to an incident. RESPONSIBILITY All personnel are responsible for understanding the circumstances under which a failure to respond should be reported. Company Officers are responsible for properly reporting all apparatus failures to respond. District Chiefs are responsible to investigate all failures to respond. The Platoon Chief is responsible for forwarding all reports of failure to respond. GUIDELINES 1. On rare occasions in which apparatus fail to respond to an incident, these circumstances shall be documented and forwarded through proper channels. Situations that might cause a failure to respond include, but are not limited to the following: a) Mechanical breakdown (failure to start). b) Injury to personnel. c) Failed communication. d) Collisions preventing response. 2. Failures to respond should be immediately reported to the TFS Communications Centre via radio or telephone for the purpose of allowing other apparatus to be dispatched as a replacement. 3. The Company Officer shall notify the respective District Chief via telephone. 4. The respective District Chief shall investigate all failures to respond and shall implement or recommend actions to prevent such a recurrence. 5. The District Chief shall immediately advise the Platoon Chief of all failures to respond that result in any of the following: a) Serious consequence. b) Potential liability against the City. c) Injury to personnel. d) Damage to apparatus or equipment. All failures to respond shall be reported by the Company Officer using e-mail in addition to any other applicable report forms relevant to the incident. 7. The e-mail shall include but not be limited to the following: a) Incident Number. a) Explanation of why the failure to respond occurred b) Company Officer’s; c) Name, d) Personnel number. 8. The Company Officer should then send the e-mail to their respective District Chief with a copy to their Platoon Chief. The Platoon Chief shall be responsible for advising the Division Commander as necessary.
