Fundamentals of Nursing 5th Lecture Ibrahim Rawhi Ayasreh RN, MSN 2011 Mr. Ibrahim Ayasrerh 2011 Implementation Implementation : is the fourth phase of nursing process, and consists of doing and documenting nursing activities or orders. Not every nursing action is directed by intervention that follows from nursing diagnosis. Mr. Ibrahim Ayasrerh 2011 Types of Nursing Skills Cognitive skills: include problem solving, critical thinking, and creativity. Interpersonal skills: all activities – verbal or nonverbal – people use when interacting directly with others. Technical skills: are “ hands-on” skills. Such as moving, giving injections. Also they are called procedures or tasks. Mr. Ibrahim Ayasrerh 2011 Process of Implementation Reassessing the client. Determining the nurse’s need for assistance. Implementing the nursing interventions. Supervising the delegated care. Documenting nursing activities. Mr. Ibrahim Ayasrerh 2011 Reassessing the client Just before implementing an intervention, the nurse must reassess the client to make sure that the intervention is still needed. For example, Ahmad has a nursing diagnosis of: “ disturbed sleep pattern”. During rounds, the nurse discovers that Ahmad is sleeping so she defer “Back – Massage” that had been planned as a relaxation technique. Mr. Ibrahim Ayasrerh 2011 Determining the Nurse’s Need for Assistance - The nurse may require assistance for one of the following reasons: * The nurse is unable to implement the nursing activity safely alone. * Assistance will reduce stress on the client. * The nurse lack a knowledge or skills to implement a particular nursing activity. Mr. Ibrahim Ayasrerh 2011 Guidelines for Implementing the nursing interventions Base nursing interventions on scientific knowledge, nursing research and professional standards of care. Clearly understand the orders to be implemented and question any that are not understood. Implement safe care. Provide teaching, support, and comfort. Be holistic. Respect the dignity of the client, and enhance self-esteem. Enhance the client to actively participate in implementing nursing interventions. Evaluating is planned, ongoing, purposeful activity in which clients and health care professionals to determine : a) client’s progress toward achievement of goals/outcomes and b) effectiveness of nursing care plan. Mr. Ibrahim Ayasrerh 2011 Process of Evaluation Collecting data related to outcomes. Comparing data with outcomes. Relating nursing activities with outcomes. Drawing conclusions about problem status.
