Foot joint complex 1. Anatomic considerations - MEDIOTARSAL OR CHOPART JOINT: - Arthrodia joint - Calcaneus + cuboid (saddle) and talus + scaphoid (bull-socket) - TARSOMETATARSAL OR LISFRANC JOINT: - Arthrodia joint - Three cuneiforms and cuboid + metatarsals - INTERMETATARSAL: - Synovials: between the metatarsals - Lateral faces of the metatarsals - METATARSOPHALANGEAL: - Condylar joint - Metatarsal heads (convex) + phalangeal bases (concave) - Flexion - extension // Abduction - adduction. - INTERPHALANGEAL: - Trochlear joint - Head of the proximal phalanx (convex) + base of the distal phalanx (concave) - Dorsiflexion - plantarflexion 2. Tests: NAVICULAR DROP TEST: - Performance: we do a mark in the scaphoid tuberosity sitting with knee and hip flexed at 90º. We ask him to stand up and we mark the scaphoid tuberosity again - Outcome: difference between both marks greater than ten millimeters - Clinical significance: pronated foot → weakness of the supinators INTRINSIC MUSCLE TEST: - Performance: patient standing and we ask him to pick up different objects from the floor with his toes and place them in a box - Outcome: impossibility of doing it or the object falls on the way - Clinical significance: weakness of the intrinsic muscles LATERAL STEP DOWN TEST: - Performance: patient standing on a step with an unipodal support only of the last four toes. We ask him to make the gesture of dropping and going up laterally from the step. - Outcome: step fall or loss of the arch of the foot - Clinical significance: pronated foot and weakness of the supinators INFLUENCE OF FOOTWEAR ON STRIDE: - Performance: patient standing and we ask him to run down the hallway, firstly, barefoot and after, in his sneakers - Outcome: forefoot strike or rearfoot strike
