Changes in body appearance or function. 2. Unusual body emissions. 3. Sensory changes. 4. Uncomfortable physical manifestations. 5. Changes in emotional status. 6. Changes in relationships Nursing process Critical thinking framework that involves assessing and diagnosis human problems to plan and implement nursing care that meets patient needs as evidenced by evaluation of patient outcomes. CHARACTERISTICS OF THE NURSING PROCESS: 1. Patient-centered. 2. Interpersonal. 3. Collaborative. 4. Dynamic and cyclical. 5. Requires critical thinking. 1. Assessment 1. The first step of the nursing process is the ongoing, systematic collection, validation, and documentation of data. 2. Nursing assessment should be comprehensive, holistic, and accurate. 3. Adequate assessment depends on collecting data using various methods, collecting both subjective and objective data, verifying that data are accurate, and communicating information about assessments to other health team members. Methods of Data Collection 1. Physical examination: The use of inspection, auscultation, percussion, and palpation to collect data about a patient’s physical status. 2. Interviewing. a. Formal approach. b. Informal approach. 3. Clinical record review. A. Involves monitoring information collected about the patient in the clinical record. B. Gathers information about results of laboratory examinations, diagnostic Procedures, consultations by other members of the health team, and progress notes. Sources of Data: 1. Primary source. 2. Secondary sources. 3. Tertiary sources. Types of Data: 1. Objective data (also known as signs). 2. Subjective data (also known as symptoms). 3. Verbal data. 4. Nonverbal data. Ineffective breathing pattern. etc. 2 -Etiology (related factor and risk factor): Identifies one or more probable causes of the health problem. 3 -Defining characteristics: Are clusters of signs & symptoms that indicate the presence of a particular? Diagnostic label. Planning 1..The third step of the nursing process provides direction for nursing interventions. It is concerned with identifying priorities, establishing goals and expected outcomes, and selecting nursing interventions that will help the patient achieve those goals and expected outcomes. 2..Planning begins when a patient is admitted and is ongoing to meet the changing or emerging needs of the patient. Communicated to all members of the health team. Implementation. The fourth step of the nursing process is the actual performance of nursing actions. Tip: If interventions are not documented, they are considered not done. Evaluation The fifth step of the nurse process, involves issues The nurse first reassesses the patient to identify patient responses to interventions (actual outcomes) and then Compares the actual outcomes with expected outcomes to
