Clinical Cases in Endocrinology Phase 2 Academic Block 1 Group Discussion August 2024 Learning outcomes To recognise the clinical presentation of four common endocrine conditions To understand the workup and basic management of these conditions Case 1 33 year old woman presents to the ED with nausea and vomiting for a week 6 months of fatigue, lack of energy and weight loss of 2 stones Reports no purging, no abdominal pain, diarrhoea, dysuria, fever or headache PMH: Diagnosed with hypothyroidism a year ago, on levothyroxine O/E: Lying in bed, pale, listless, dehydrated, BP 93/60, pulse 112 bpm Chest and abdomen unremarkable Case 1: Blood results Results Normal range Sodium 128 umol/L 133 - 146 Potassium 5.7 mmol/L 3.5 - 5.3 Urea 16.7 mmol/L 2.5 – 7.8 Creatinine 82 umol/L 45 - 84 Plasma glucose 2.2 mmol/l 4 - 11 Amylase 42 IU/L 30 - 118 Examination Case 1 – Questions (Investigations) 1. Case 2 45 year old woman presents to the ED with high fever, agitation and confusion She has a history of weight loss of 7 kg and diarrhoea over 3 months. Also C/O palpitations, somnolence and restlessness over few days. PMH: Type 2 DM, on metformin, previous C-section x2 Clinical examination: Agitated patient. Sweaty palms. Observations: temp 40oC, BP 160/100, pulse 145 bpm, RR 18/min Bloods results awaited ECG Blood results Result Normal range Sodium 135 umol/L 133 - 146 Potassium 3.8 mmol/L 3.5 - 5.3 Urea 9.2 mmol/L 2.5 – 7.8 Creat 110 umol/L 45 - 84 Protein 65 g/L 60 - 80 Albumin 42 g/L 35 - 50 Bilirubin 10 umol/L
